摘要
目的探讨柴胡清肝汤加减对肝郁化热型肿块期浆细胞性乳腺炎(plasma cell mastitis,PCM)临床症状及炎症指标的影响。方法选取2021年1月至2023年6月济宁市中医院外一科门诊明确诊断为肝郁化热型肿块期PCM的80例患者作为研究对象,按随机数表法分为对照组与治疗组,每组40例。对照组予以甲泼尼龙片和阿莫西林胶囊治疗,治疗组在对照组的基础上加用柴胡清肝汤加减治疗,比较两组临床症状积分、炎症指标、免疫指标和催乳素水平。结果治疗组治疗总有效率为97.50%,高于对照组的87.50%(P<0.05);治疗后,两组中医症状积分均低于治疗前,且治疗组低于对照组(P<0.05);治疗后,两组PCT、CRP、IL-6、ESR水平低于治疗前,且治疗组低于对照组(P<0.05);治疗后,两组IgA、IgG、IgM、PRL水平均低于治疗前,且治疗组低于对照组(P<0.05)。结论柴胡清肝汤加减对肝郁化热型肿块期PCM疗效显著,可以明显消除炎症、改善免疫、缓解症状,提高患者生命质量,值得临床推广应用。
Objective To investigate the effect of modified Chaihu Qinggan decoction on the clinical symptoms and inflammatory indicators of plasma cell mastitis(PCM)in the mass stage of liver depression with heat transformation type.Methods 80 patients diagnosed with PCM in the mass stage of liver depression with heat transformation type in the Outpatient department of the Department of External Medicine of Jining Traditional Chinese Medicine Hospital from January 2021 to June 2023 were selected as the research subjects,and they were divided into the control group and the treatment group using the random number table method,with 40 cases in each group.The control group was treated with methylprednisolone tablets and amoxicillin capsules,while the treatment group was supplemented with Chaihu Qinggan decoction on the basis of the control group.The clinical symptom scores,inflammation indicators,immune indicators,and prolactin levels of the two groups were compared.Results The total treatment effective rate in the treatment group was 97.50%,which was higher than 87.50%in the control group(P<0.05).After treatment,the Traditional Chinese Medicine symptom scores of the two groups were lower than those before treatment,and the treatment group was lower than the control group(P<0.05).After treatment,the PCT,CRP,IL-6,ESR of the two groups were lower than those before treatment,and the treatment group was lower than the control group(P<0.05).After treatment,the IgA,IgG,IgM,PRL of the two groups were lower than those before treatment,and the treatment group was lower than the control group(P<0.05).Conclusion Chaihu Qinggan decoction with modifications has good clinical efficacy in the treatment of PCM in the mass stage of liver depression with heat transformation type,it can significantly eliminate inflammation,improve immunity,alleviate symptoms,and improve the quality of life of patients,it is worthy of clinical promotion and application.
