摘要
目的:探讨沙库巴曲缬沙坦联合达格列净治疗心力衰竭(HF)疗效及对患者心肌损伤、血流动力学的影响。方法:选取2022年1月—2024年1月开封市人民医院医学心内科收治的92例HF患者作为研究对象。采用随机数表法分为对照组和研究组,每组各46例。比较两组患者临床疗效和治疗前后血清N末端脑钠肽前体(NT-proBNP)、肌酸激酶同工酶(CK-MB)和心肌肌钙蛋白I(cTnI)、左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、六分钟步行试验(6MWT)、心输出量(CO)、心脏指数(CI)和心缩力指数(HI)及不良反应发生率、主要心脏不良事件(MACE)发生率。结果:研究组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗后,研究组患者血清NT-ProBNP、cTnI、CK-MB和LVEDD低于对照组,LVEF、6MWT和血清CO、CI、HI均高于对照组,差异均有统计学意义(P<0.001);两组患者不良反应事件发生率与MACE发生率比较,差异均无统计学意义(P>0.05)。结论:沙库巴曲缬沙坦联合达格列净可提高HF患者临床疗效,降低心肌损伤,稳定血流动力学,安全性好。
Objective:To explore the curative effect of sacubitril valsartan combined with dapagliflozin and its influences on myocardial injury and hemodynamics in patients with heart failure(HF).Methods:A total of 92 patients with HF admitted to Cardiovascular Department of Kaifeng People's Hospital were enrolled from January 2022 to January 2024.According to random number table method,they were divided into control group and study group,with 46 cases in each group.The clinical curative effect,serum N-terminal pro-brain nitric peptide(NT-proBNP),creatine kinase isoenzymes(CK-MB),cardiac troponin I(cTnI),left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),6-minute walking test(6MWT),cardiac output(CO),cardiac index(CI)and heather index(HI)before and after treatment,incidence of adverse reactions and major adverse cardiac events(MACE)were compared between the two groups.Results:Compared with control group,total response rate of treatment was higher in study group,with statistical difference(P<0.05).After treatment,serum NT-ProBNP,cTnI,CK-MB and LVEDD in study group were lower than those in control group,while LVEF,6MWT,serum CO,CI and HI were higher than those in control group,with statistical difference(P<0.001).There was no significant difference in the incidence of adverse reactions or MACE between the two groups,with statistical difference(P>0.05).Conclusion:Sacubitril valsartan combined with dapagliflozin can improve clinical curative effect,reduce myocardial injury and stabilize hemodynamics in HF patients,with good safety.
