摘要
川芎-香附是临床上经典的理气活血药对,首次出现于《丹溪心法》,具有疏肝解郁、理气活血止痛的功效,除柴胡疏肝散、越鞠丸等经典名方见此配伍外,该药对也被应用于多种中成药中,在临床用药中发挥重要作用。现代研究发现,川芎-香附药对富含挥发油、生物碱等有效成分,两药协同配伍,在不同配比条件下,该药对的药代动力学及药效存在差异,与单味药材相比,药对的指标成分含量均有所增加。药理学研究表明,川芎-香附药对通过调节神经活性配体-受体相互作用、PI3K-Akt信号通路、多巴胺能突触和mTOR信号通路、NOS/NO通路等多通路、多途径起到抗抑郁、抗偏头痛、调节血液循环、抑制子宫收缩等作用,在临床上被广泛用于治疗各类妇科疾病、神经系统疾病、心脑血管疾病、消化系统疾病、内分泌疾病、肿瘤等,临床疗效确切,较之西药具有更高的安全性。综合现有文献,笔者从化学成分、药理作用及临床应用等方面出发,综述近年来川芎-香附药对的研究现状,为该药对的应用提供理论依据。
The drug pair of Chuanxiong(Rhizoma Chuanxiong)and Xiangfu(Rhizoma Cyperi)is a classic Qi-regulating and blood-activating drug pair in clinical practice.It first appeared in Danxi's Experiential Therapy.It has the effects of soothing the liver and relieving depression,regulating Qi,promoting blood circulation and relieving pain.Besides being found in classic prescriptions such as Chaihu Shugan San(柴胡疏肝散)and Yueju Wan(越鞠丸),this drug pair is also applied in a variety of proprietary Chinese medicines and plays an important role in clinical medication.Modern research has found that the drug pair of Chuanxiong(Rhizoma Chuanxiong)and Xiangfu(Rhizoma Cyperi)is rich in effective components such as volatile oils and alkaloids.When the two drugs are synergistically combined,under different proportion conditions,the pharmacokinetics and pharmacodynamics of this drug pair are different.Compared with single medicinal materials,the contents of the index components of the drug pair are all increased.Pharmacological studies have shown that the drug pair of Chuanxiong(Rhizoma Chuanxiong)and Xiangfu(Rhizoma Cyperi)exerts effects such as anti-depression,anti-migraine,regulating blood circulation,and inhibiting uterine contraction through multiple pathways and multiple approaches,including regulating the interaction between neuroactive ligands and receptors,the PI3K-Akt signaling pathway,the dopaminergic synapse,the mTOR signaling pathway,and the NOS/NO pathway.Clinically,it is widely used in the treatment of various gynecological diseases,nervous system diseases,cardiovascular and cerebrovascular diseases,digestive system diseases,endocrine diseases,tumors,etc.,with definite clinical efficacy.Compared with Western medicines,it has higher safety.Based on the existing literature comprehensively,the author reviews the research status of the drug pair of Chuanxiong(Rhizoma Chuanxiong)and Xiangfu(Rhizoma Cyperi)in recent years from aspects such as chemical components,pharmacological effects and clinical applications,so as to provide a theoretical basis for the application of this drug pair.
