摘要
目的:探讨子宫内膜癌术后辅助化疗患者的营养状况及其影响因素。方法:选取2023年3月至2025年3月于南阳市中心医院进行术后辅助化疗的104例子宫内膜癌患者为研究对象,根据患者主观整体营养状况评估量表(PG–SGA)评分将其分为营养状况良好组(58例,总分<4分)和营养不良组(46例,总分≥4分)。比较两组患者的一般资料及营养指标[血清总蛋白(TP)、白蛋白(ALB)、前白蛋白(PAB)水平],采用Logistic回归分析影响子宫内膜癌术后辅助化疗患者营养状况的因素。结果:两组患者年龄、体质量指数(BMI)、化疗分期、是否合并糖尿病、是否有胃肠道反应、应对方式、味觉改变占比比较,差异具有统计学意义(P<0.05);营养不良组患者血清TP、ALB、PAB水平均低于营养状况良好组,差异具有统计学意义(P<0.05);多因素Logistic回归分析显示,化疗晚期、合并糖尿病、消极应对方式是影响子宫内膜癌术后辅助化疗患者营养状况的独立危险因素(P<0.05)。结论:化疗晚期、合并糖尿病、消极应对方式是影响子宫内膜癌术后辅助化疗患者营养状况的独立危险因素。
Objective To explore the nutritional status and influencing factors of patients with endometrial cancer after adjuvant chemotherapy.Methods A total of 104 patients with endometrial cancer who received adjuvant chemotherapy at Nanyang Central Hospital from March 2023 to March 2025 were selected as the research subjects.According to the patient-generated subjective global assessment(PG-SGA)score,they were divided into a good nutritional status group(58 cases,total score<4)and a malnutrition group(46 cases,total score≥4).The general data and nutritional indicators[serum total protein(TP),albumin(ALB),prealbumin(PAB)levels]of the two groups were compared,and Logistic regression analysis was used to analyze the factors influencing the nutritional status of patients with endometrial cancer after adjuvant chemotherapy.Results There were statistically significant differences in age,body mass index(BMI),chemotherapy stage,whether diabetes was combined,whether there were gastrointestinal reactions,coping styles,and the proportion of taste changes between the two groups(P<0.05);the levels of serum TP,ALB,and PAB in the malnutrition group were lower than those in the good nutritional status group,and the differences were statistically significant(P<0.05);multivariate Logistic regression analysis showed that advanced chemotherapy stage,combined diabetes,and negative coping style were independent risk factors affecting the nutritional status of patients with endometrial cancer after adjuvant chemotherapy(P<0.05).Conclusion Advanced chemotherapy stage,combined diabetes,and negative coping style are independent risk factors affecting the nutritional status of patients with endometrial cancer after adjuvant chemotherapy.
