摘要
Ⅳ型心肾综合征是指由原发性慢性肾脏病导致心功能障碍的一种临床综合征。在疾病的发生与发展过程中,其表现出“肾气不化、肾不藏精、心阳虚衰”的病机特点,与“水气凌心”证的临床表现高度契合。基于“温肾化气法”,临证提出以“水气凌心”作为核心病机辨治Ⅳ型心肾综合征。文章以“水气凌心”为核心病机阐述Ⅳ型心肾综合征的发病机制,并根据其病机提出以温肾化气法为治则,采用“温肾化气、温补心阳、化气行水、交通心肾”的治疗方法治疗Ⅳ型心肾综合征。并列举一则临证验案,以期为临床辨治此类疾病提供参考。
TypeⅣcardio-renal syndrome(CRS)refers to a clinical syndrome of cardiac dysfunction secondary to primary chronic kidney disease.During the occurrence and development of the disease,it shows the pathogenesis characteristics of“kidney Qi(气)failing to transform fluid,kidney failing to store essence,heart Yang(阳)deficiency and failure”,which is highly consistent with the clinical manifestationsof the“water Qi attacking the heart”syndrome(水气凌心证)in traditional Chinese medicine.Based on the theory of“warming kidney to transform Qi”,this article proposes to treat typeⅣCRS with the theory of“water Qi attacking the heart”as the core pathogenesis.This paper elaborates on the pathogenesis of typeⅣCRS from the persipective of“water Qi attacking the heart”,and puts forward the treatment principle of“warming kidney to transform Qi”according to its pathogenesis,and adopts the treatment method of“warming the kidney to transform Qi,warming and tonifying the heart Yang,transforming Qi to circulate water,and connecting the heart and kidney”to treat typeⅣCRS.In addition,is presentied to provide reference for the clinical diagnosis and treatment of such diseases.
