摘要
目的:基于中医传承辅助系统分析中国知网中以中药联合放化疗治疗食管癌的处方规律并发掘新处方。方法:检索中国知网中自2000―2025年收录的关于中药联合放化疗治疗食管癌的相关文献,按照纳入与排除标准对文献进行筛选,将纳入文献中的处方录入中医传承辅助系统平台软件(V2.5)并对其进行组方规律分析,在核心药物组合的基础上发掘新处方。结果:共有155首方剂符合纳入要求,得到240味中药,频次≥15的中药共36味,以补虚药、清热药、活血化瘀药、化痰药为主,基于关联规则得到常用药对44对,白术-茯苓频次最高,基于熵聚类分析得到药物核心组合28对,新处方14个,并将得到的新处方与传统方剂和相关医家的处方经验进行分析。结论:中药联合放化疗治疗食管癌以扶正为主,祛邪为辅,健脾益气滋阴、活血化瘀、化痰降逆、清热解毒为其基本治法。
Objective:To analyze prescription principles and discover new prescriptions of traditional Chinese medicine(TCM)combined with radiotherapy and chemotherapy for esophageal cancer,based on literature collected from the China National Knowledge Infrastructure(CNKI)and analyzed using the Traditional Chinese Medicine Inheritance Support System(TCMISS).Methods:Relevant literature on the treatment of esophageal cancer with TCM combined with radiotherapy and chemotherapy was retrieved from CNKI for the period 2000 to 2025.The literature was screened according to inclusion and exclusion criteria.Prescriptions included in the literature were entered into the TCMISS platform software(v2.5)and analyzed to uncover prescription patterns and to explore new prescriptions based on core drug combinations.Results:A total of 155 prescriptions met the inclusion requirements,yielding 240 distinct Chinese medicines.Thirty-six Chinese medicines were used with a frequency greater than or equal to 15 times,primarily including tonifying deficiency drugs,heat-clearing drugs,blood-activating and stasis-removing drugs,and phlegm-resolving drugs.Based on association rules,44 pairs of commonly used coupled medicinals were obtained,with Baizhu(Rhizoma Atractylodis Macrocephalae)-Fuling(Poria)having the highest frequency.Based on entropy cluster analysis,28 core drug combinations and 14 new prescriptions were obtained.These new prescriptions were analyzed and compared with traditional prescriptions and the prescription experiences of related physicians.Conclusion:The treatment of esophageal cancer with TCM combined with radiotherapy and chemotherapy is based on strengthening the body and supplemented by eliminating pathogens.The fundamental treatment methods include strengthening the spleen,nourishing Qi(气)and Yin(阴),promoting blood circulation and removing blood stasis,reducing phlegm and adverse effects,and clearing heat and detoxification.
