摘要
目的探究子宫内膜恶性肿瘤患者实施前哨淋巴结(sentinel lymph nodes,SLN)切除术的效果。方法选取2022年1月至2023年8月新余市人民医院收治的48例子宫内膜癌患者作为研究对象,所有患者均于手术开始前注射亚甲蓝2 ml,手术开腹或腹腔镜探查,识别蓝染淋巴结定位为SLN,单独切除SLN后行标准子宫内膜癌分期术。统计SLN分布情况、检出情况及病理检查的诊断效能。结果48例子宫内膜癌患者髂外血管区分布更为常见,占比为43.75%,其次为闭孔区,占比为18.75%。以盆腔和/或腹主动脉旁淋巴结病理结果作为金标准,结果显示,阳性47例,阴性1例;SLN病理结果诊断子宫内膜癌的准确度为93.75%,灵敏度为95.74%,阴性预测值为0.00%,假阴性率为4.25%。结论亚甲蓝示踪技术可有效识别SLN,且SLN切除术在子宫内膜癌患者中的诊断效能较高,为淋巴结清扫的范围提供重要的参考依据。
Objective To investigate the effect of sentinel lymph node(SLN)resection in patients with endometrial malignancy.Methods 48 patients with endometrial cancer admitted to Xinyu People's Hospital from January 2022 to August 2023 were selected as the research subjects.All patients were injected with 2 ml of methylene blue before the start of the surgery,and the surgery was performed through laparotomy or laparoscopy to identify the lymph nodes as SLN.After the SLN was removed alone,standard endometrial cancer staging surgery was performed.The distribution and detection rate of SLN,and diagnostic performance were analyzed.Results Among the 48 endometrial cancer patients,the external iliac vessel area was the most common site for SLN distribution,accounting for 43.75%,followed by the obturator area at 18.75%.Using the pathological results of pelvic and/or para-aortic lymph nodes as the gold standard,47 cases were positive and 1 case was negative.The pathological results of SLN showed a diagnostic accuracy of 93.75%,sensitivity of 95.74%,negative predictive value of 0.00%,and a false-negative rate of 4.25%for endometrial cancer.Conclusion Methylene blue tracing technique can effectively identify SLN,and SLN resection demonstrates high diagnostic performance in patients with endometrial carcinoma,providing an important reference for determining the extent of lymph node dissection.
