摘要
“主客交”理论出自《瘟疫论》,原用于指导疫病后期的诊疗,经后世医家发展,现可用于指导治疗多种慢性迁延性顽疾。陆为民教授基于“主客交”理论辨治溃疡性结肠炎(UC),常从“主气不足”与“客气交浑”入手,认为“主气不足”即脾气虚馁是UC的核心病机,湿痰瘀邪等“客气交浑”、久羁肠腑是本病缠绵难愈之兼夹病机。基于上述理论陆师治疗UC,强调补益脾气以复其主气,祛湿、化痰、逐瘀以蠲其客气,动态权衡主客之性,补虚顾及阴阳之偏,祛邪明辨邪侵之实,精准施治,收效颇佳。
The theory of"Interaction between Host and vip Qi"originates from Treatise on Pestilence,originally used to guide the diagnosis and treatment in the later stages of epidemic diseases.Developed by later physicians,it can now be used to guide the treatment of various chronic and protracted refractory diseases.Based on the theory of"Interaction between Host and vip Qi",Professor LU Weimin's approach to the differentiated treatment of ulcerative colitis(UC)often stars from"insufficiency of host qi"and"intermingling of vip qi".He believes that"insufficiency of host qi",namely Spleen Qi deficiency,is the core pathogenesis of UC,while the"intermingling of vip qi"such as dampness,phlegm,and stasis pathogens,lingering in the intestines for a long time,is the concomitant pathogenesis that makes this disease persistent and difficult to cure.Based on the above theory,Professor LU's treatment of UC emphasizes tonifying the Spleen Qi to restore the host qi,and eliminating dampness,resolving phlegm,and dispelling stasis to remove the vip qi.He believes that therapeutic strategies should aim to balance the nature of the host and vip dynamically.In tonifying deficiency,the imbalance between yin and yang should be taken into account;while in eliminating pathogens,the reality of the pathogen invasion should be distinguished.Precise treatment can achieve remarkable results.
