血清PTEN、ApoB/ApoA-Ⅰ及精浆活性氧水平与不育男性患者精液质量的相关性

摘要 目的探讨血清磷酸酶和张力蛋白同源物(PTEN)、载脂蛋白B/载脂蛋白A-Ⅰ(ApoB/ApoA-Ⅰ)、精浆活性氧(ROS)水平与不育男性患者精液质量的相关性。方法选取2024年1月至12月温州市人民医院收治的98例不育男性患者作为研究对象,设为不育组。另选取同期100例生育功能正常男性设为对照组。比较两组的精液质量参数(精子浓度、精子总活力、前向运动精子百分率、精子平均曲线运动速度、精子平均直线运动速度、精子平均路径运动速度、精子头部侧向平均振幅)、血清PTEN、ApoB、ApoA-Ⅰ、精浆ROS,采用Pearson相关分析法分析精液质量参数与PTEN、ApoB/ApoA-Ⅰ、精浆ROS的相关性。结果不育组精子浓度、精子总活力、前向运动精子百分率、精子平均曲线运动速度、精子平均直线运动速度、精子平均路径运动速度、精子头部侧向平均振幅均显著低于对照组,差异具有统计学意义(P<0.05);不育组血清PTEN水平显著低于对照组,血清ApoB、ApoB/ApoA-Ⅰ、精浆ROS水平均显著高于对照组,差异具有统计学意义(P<0.05)。血清PTEN与不育男性患者的精子浓度、精子总活力、前向运动精子百分率、精子平均曲线运动速度、精子平均直线运动速度、精子平均路径运动速度、精子头部侧向平均振幅呈显著正相关(P<0.05);血清ApoB/ApoA-Ⅰ、精浆ROS均与不育男性患者的精子浓度、精子总活力、前向运动精子百分率、精子平均曲线运动速度、精子平均直线运动速度、精子平均路径运动速度、精子头部侧向平均振幅呈显著负相关(P<0.05)。结论不育男性患者血清PTEN水平显著降低,ApoB/ApoA-Ⅰ、精浆ROS水平显著升高,且上述指标与精液质量参数有密切的相关性。 Objective To explore the correlations between the levels of serum phosphatase and tensin homolog(PTEN),apolipoprotein B/apolipoprotein A-Ⅰ(ApoB/ApoA-Ⅰ),and seminal plasma reactive oxygen species(ROS)levels and semen quality in male infertile patients.Methods A total of 98 male infertile patients admitted to Wenzhou People′s Hospital from January to December 2024 were selected as research subjects and set as the infertility group.Another 100 men with normal reproductive function during the same period were selected as the control group.The semen quality parameters(sperm concentration,total sperm motility,percentage of forward motility sperm,average curvilinear movement speed of sperm,average linear movement speed of sperm,average path movement speed of sperm,average lateral amplitude of sperm head),serum PTEN,ApoB,ApoA-Ⅰ,and seminal plasma ROS were compared between the two groups.Pearson correlation analysis method was used to analyze the correlations between semen quality parameters with PTEN,ApoB/ApoA-Ⅰ,and seminal plasma ROS.Results The sperm concentration,total sperm motility,percentage of forward-moving sperm,average curvilinear sperm movement speed,average linear sperm movement speed,average path sperm movement speed,and average lateral amplitude of sperm head in the infertility group were all significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The serum PTEN level in the infertility group was significantly lower than that in the control group,while the serum ApoB,ApoB/ApoA-Ⅰ,and seminal plasma ROS levels were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Serum PTEN was significantly positively correlated with sperm concentration,total sperm motility,percentage of forward-moving sperm,average curvilinear sperm movement speed,average linear sperm movement speed,average path sperm movement speed,and average lateral amplitude of sperm head in male infertile patients(P<0.05).Serum ApoB/ApoA-Ⅰand seminal plasma ROS were significantly negatively correlated with sperm concentration,total sperm motility,percentage of forward motility sperm,average curvilinear sperm motility speed,average linear sperm motility speed,average path sperm motility speed,and average lateral amplitude of sperm head in male infertile patients(P<0.05).Conclusions The serum PTEN level of male infertile patients is significantly decreased,while the ApoB/ApoA-Ⅰand seminal plasma ROS levels are significantly increased.Moreover,the above indicators are closely correlated with semen quality parameters.
