摘要
目的探讨多层螺旋CT在小儿肺炎支原体肺炎及合并肺炎链球菌性肺炎诊断中的价值。方法选取2023年1月—2024年4月收治的76例肺炎支原体肺炎和肺炎支原体肺炎合并肺炎链球菌性肺炎患儿进行研究分析,其中肺炎支原体肺炎患儿40例,肺炎支原体肺炎合并肺炎链球菌性肺炎患儿36例。所有患儿均接受多层螺旋CT检查。分析两组患儿多层螺旋CT表现中气道与肺间质病变征象、肺内病变形态及胸腔积液、淋巴结最大横径与胸腔积液前后最厚处测量值。结果与合并感染组比较,单纯支原体感染组支气管壁增厚、网状影、磨玻璃影、支气管血管束增厚的占比高(P均<0.05)。单纯支原体感染组患儿中,出现扇形薄片影的比例为72.50%,高于合并感染组的13.89%;出现无规律肺内实变影的比例为25.00%,显著低于合并感染组的66.67%(P均<0.05)。合并感染组患儿胸腔积液的占比为55.56%,相较于单纯支原体感染组(30.00%)升高,且胸腔厚度、淋巴结最大横径均比单纯支原体感染组高(P均<0.05)。结论多层螺旋CT检查应用于小儿肺炎支原体肺炎及合并肺炎链球菌性肺炎的临床诊断有一定的应用价值。
Objective To explore the clinical value of Multi-Slice Spiral CT(MSCT)in distinguishing Mycoplasma Pneumoniae Pneumonia(MP)and MP combined with Streptococcus Pneumoniae Pneumonia(SP)in children.Methods A retrospective analysis was conducted on 76 children with MP and MP combined with SP admitted from January 2023 to April 2024.Among them,40 cases were diagnosed with MP,and 36 cases were MP combined with SP.All patients underwent MSCT examination.Key imaging features,including airway and pulmonary interstitial lesions,morphology of pulmonary lesions,and measurements of the maximum transverse diameter of pleural effusion and lymph nodes,as well as the thickest point before and after pleural effusion,were analysed and compared between the two groups.Results Compared with the MP combined with SP group,the pure MP group exhibited higher incidences of bronchial wall thickening,reticular opacities.ground glass opacities,and bronchial vascular bundle thickening(all P<0.05).In the MP group,29 cases(72.50%)showed fan-shaped thin film shadows,contrasting with 13.89%in MP combined with SPgroup.A total of 10 cases(25.00%)showed irregular pulmonary consolidation shadows,while 24 cases(66.67%)were observed in the MP combined with SP group.The MP combined with SP group showed higher prevalence of pleural effusion(55.56%vs.30.00%)and larger pleural thickness and lymph node diameters compared to the MP group(both P<0.O5).Conclusion MSCT examination demonstrates significant clinical application value in the differentiation of MP and MP combined with SP in children.
