摘要
目的探讨基于物联网云平台管理的家庭无创通气在慢性阻塞性肺疾病(COPD)合并慢性呼吸衰竭患者中的应用效果。方法选取北京中医药大学房山医院于2020年2月至2022年2月收治的90例COPD合并慢性呼吸衰竭患者为研究对象,根据患者就诊顺序,通过计算机随机数字表法将其分为观察组(45例)和对照组(45例)。对照组进行常规家用呼吸机管理模式,观察组进行基于物联网云平台的家庭无创通气管理模式。比较2组患者的治疗依从性、肺功能、慢性阻塞性肺疾病评估测试(CAT)评分、呼吸困难程度(mMRC)评分。结果观察组平均每日用机时长长于对照组,每日用机时长>8 h的天数百分比高于对照组,漏气量明显少于对照组,差异均有统计学意义(P均<0.05)。干预后,观察组的第1秒用力呼气容量(FEV1)、第1秒用力呼气容量与用力肺活量的比值(FEV1/FVC)均明显高于对照组(P均<0.05);观察组的CAT评分及mMRC评分均低于对照组(P均<0.05)。结论在COPD合并慢性呼吸衰竭患者的治疗当中,采用基于物联网云平台的家庭无创通气管理,临床优势十分明显,能够改善患者的肺功能、减轻呼吸困难症状,提高呼吸机治疗依从性和生活质量。
Objective To investigate the application effect of home noninvasive ventilation management based on the Internet of Things cloud platform in patients with chronic obstructive pulmonary disease(COPD)complicated by chronic respiratory failure.Methods A total of 90 COPD patients with chronic respiratory failure admitted to Fangshan Hospital,Beijing University of Chinese Medicine from February 2020 to February 2022 were selected as the subjects of study.According to the order of patientsvisits,they were allocated into an observation group(45 cases)and a control group(45 cases)by using a computer generated random number table.The control group received the conventional home ventilator management,while the observation group received the home noninvasive ventilation management based on the Internet of Things cloud platform.The treatment compliance,pulmonary function,COPD Assessment Test(CAT)score,and modified Medical Research Council(mMRC)score of the two groups were compared.Results The average daily duration of machine usage in the observation group exceeded that in the control group.The percentage of days with daily duration of machine usage>8 h in the observation group was higher than that in the control group.The air leakage was significantly less than that in the control group.The differences were all statistically significant(P<0.05).After intervention,the forced expiratory volume in 1 second(FEV1)and forced expiratory volume in 1 second/forced vital capacity(FEV1/FVC)in the observation group were significantly higher than those in the control group(P<0.05).The CAT and mMRC scores of the observation group were both lower than those of the control group(P<0.05).Conclusion In the treatment of COPD patients with chronic respiratory failure,the adoption of home noninvasive ventilation management based on the Internet of Things cloud platform has obvious clinical advantages,which can improve patientspulmonary function,alleviate their dyspnea symptoms,and improve their ventilator treatment compliance and quality of life.
