摘要 目的:探讨无创高频振荡通气(nHFOV)联合枸橼酸咖啡因对新生儿呼吸窘迫综合征(NRDS)患儿动脉血气指标及炎症指标的影响。方法:选取2022年5月—2024年3月莆田学院附属医院收治的NRDS患儿88例,按随机数字表法分为对照组、研究组,各44例。对照组予以nHFOV治疗,研究组予以nHFOV联合枸橼酸咖啡因治疗。对比两组治疗效果、动脉血气指标、炎症指标、呼吸机治疗时间、总住院时间及并发症发生率。结果:研究组治疗总有效率较对照组高(P<0.05);治疗5 d后,研究组SaO_(2)、PaO_(2)较对照组高,PaCO_(2)、CRP、TNF-α、降钙素原(PCT)较对照组低(P<0.05);研究组呼吸机治疗时间、总住院时间均较对照组短(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:NRDS患儿采用nHFOV联合枸橼酸咖啡因治疗效果较好,可改善动脉血气指标,降低炎症反应,缩短呼吸机使用及住院时间。 Objective:To investigate the effects of noninvasive high-frequency oscillatory ventilation(nHFOV)combined with Caffeine Citrate on arterial blood gas indexes and inflammatory indexes in children with neonatal respiratory distress syndrome(NRDS).Method:A total of 88 children with NRDS admitted to the Affiliated Hospital of Putian University from May 2022 to March 2024 were selected and divided into the control group and the study group according to the random number table method,with 44 cases in each group.The control group was treated with nHFOV,while the study group was treated with nHFOV combined with Caffeine Citrate.The therapeutic effects,arterial blood gas indexes,inflammatory indexes,ventilator treatment time,total hospital stay and incidence of complications were compared between the two groups.Result:The total treatment efficiency of the study group was higher than that of the control group(P<0.05).After 5 days of treatment,the SaO_(2) and PaO_(2) of the study group were higher than those of the control group,the PaCO_(2),CRP,TNF-αand procalcitonin(PCT)of the study group were lower than those of the control group(P<0.05).The ventilator treatment time and total hospital stay of the study group were shorter than those of the control group(P<0.05).There was no statistically significant difference in the comparison of complications between the two groups(P>0.05).Conclusion:Children with NRDS are better treated with nHFOV combined with Caffeine Citrate,which can improve arterial blood gas indexes,reduce inflammatory response,and shorten ventilator use and hospital stay.
