摘要
目的探究磁共振成像(MRI)参数联合血清微小RNA-103(miR-103)、微小RNA-375(miR-375)在乳腺癌术前诊断中的应用价值。方法回顾性分析河南省直第三人民医院就诊且接受术前MRI及血清miR-103、miR-375检测的298例疑似乳腺癌患者的临床资料,根据病理结果分为良性组(n=248)和恶性组(n=50),恶性组进一步分为Ⅰ~Ⅱ期亚组(n=21)、Ⅲ期亚组(n=29),比较良性组与恶性组及恶性组不同亚组术前MRI参数及血清miR-103、miR-375水平,采用受试者工作特征(ROC)曲线分析术前各参数对乳腺癌的诊断及分期效能。结果恶性组的表观弥散系数(ADC)、miR-103、miR-375低于良性组(P<0.05);恶性组转运常数(Ktrans)、速率常数(Kep)高于良性组(P<0.05)。经ROC分析,术前MRI参数及血清miR-103、miR-375水平联合鉴别乳腺癌良恶性的ROC曲线下面积(AUC)值为0.976,联合分期诊断的AUC为0.934。结论MRI参数联合血清miR-103、miR-375检测在乳腺癌术前诊断和分期中具有重要的应用价值。
Objective To explore the application value of magnetic resonance imaging(MRI)parameters combined with serum microRNA-103(miR-103)and microRNA-375(miR-375)in preoperative diagnosis of breast cancer.Methods The clinical data of 298 patients with suspected breast cancer who received preoperative MRI and serum miR-103 and miR-375 detection in the Third People’s Hospital of Henan Province were retrospectively analyzed.According to the pathological results,the patients were divided into benign group(n=248)and malignant group(n=50).The patients in malignant group were further divided into stage I-II subgroup(n=21)and stage III subgroup(n=29).The preoperative MRI parameters and levels of serum miR-103 and miR-375 were compared between benign group and malignant group and between different subgroups of malignant group.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficiency and staging efficiency of preoperative various parameters on breast cancer.Results The apparent diffusion coefficient(ADC),miR-103 and miR-375 in malignant group were lower than those in benign group(P<0.05).The transport constant(Ktrans)and rate constant(Kep)were higher in malignant group than those in benign group(P<0.05).According to ROC analysis,the area under the ROC curve(AUC)of combination of preoperative MRI parameters,serum miR-103 and miR-375 for identifying benign and malignant breast cancer was 0.976,and the AUC of combined staging diagnosis was 0.934.Conclusion MRI parameters combined with serum miR-103 and miR-375 detection has important application value in the preoperative diagnosis and staging of breast cancer.
