Epidemiological analysis of bacterial infection in acute exacerbation of chronic obstructive pulmonary disease

摘要 目的分析慢性阻塞性肺疾病急性加重期(AECOPD)住院患者的人群特征、感染细菌分布以及耐药情况,为临床抗菌药物的使用提供建议。方法收集2020年6月1日至2023年2月1日泸州市某医院AECOPD住院患者的痰标本,进行细菌培养及药敏试验。分析不同性别、年龄患者常见细菌分布情况;分析患者常见细菌耐药情况。结果共纳入271例AECOPD患者标本,其中革兰氏阴性菌占95.94%,男性为主要感染人群(70.11%),感染者年龄以60~80岁最多。在革兰氏阴性菌中,男性不同细菌占比高于女性(P<0.05)。鲍曼不动杆菌对哌拉西林/他唑巴坦耐药率为100.00%,对其余临床常用抗生素的耐药率均≥50.00%。肺炎克雷伯杆菌对替加环素敏感率为100.00%。嗜麦芽寡养单胞菌对头孢他啶耐药率为54.29%。铜绿假单胞菌对妥布霉素、美罗培南、阿米卡星的敏感率均>90.00%。结论AECOPD患者群体呈现高龄化特征,革兰氏阴性菌是其主要感染菌。建议在经验治疗基础上,结合痰培养结果给予对应的抗菌药物治疗,以提高患者生存质量。 Objective To analyze the population characteristics,infection bacterial distribution and drug resistance of hospitalized patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),and to provide suggestions for the use of clinical antibiotics.Methods Sputum samples of AECOPD hospitalized patients in a hospital in Luzhou city from June 1,2020 to February 1,2023 were collected for bacterial culture and drug sensitivity test.The distribution of common bacteria in patients of different genders and ages was analyzed;the durg common bacterial resistance of patients was analyzed.Results Among 271 samples of AECOPD patients,Gram-negative bacteria accounted for 95.94%,and men were the main infected population(70.11%),the most infected people were 60-80 years old.Among the Gram-negative bacteria,the proportion of different bacteria in men was higher than that in women(P<0.05).The resistance rate of Acinetobacter baumannii to piperacillin/tazobactam was 100.00%,and the resistance rate to other commonly used antibiotics was≥50.00%.The sensitivity rate of Klebsiella pneumoniae to tigecycline was 100.00%.The resistance rate of Stenotrophomonas maltophilia to ceftazidime was 54.29%.The sensitivity rates of Pseudomonas aeruginosa to tobramycin,meropenem and amikacin were all>90.00%.Conclusion AECOPD patients are characterized by aging,and Gram-negative bacteria are the main infectious bacteria.It is recommended to give corresponding antibacterial drugs on the basis of empirical treatment combined with sputum culture results to improve the quality of life of patients.
