摘要
目的研究血清同型半胱氨酸(Hcy)、神经丝轻链蛋白(NFL)与帕金森病(PD)患者病程和疾病严重程度间关系。方法选择2018年5月至2020年5月郑州大学附属郑州中心医院收治的80例PD患者作为研究对象,根据其病程及疾病严重程度分为2个亚组,其中按病程分组:A组(病程<5 a,44例)、B组(5 a≤病程<9 a,25例)、C组(病程≥9 a,11例)。疾病严重程度参照Hoehn-Yahr分级评分量表评估,即:早期组(Ⅰ~Ⅱ期,33例)、中晚期组(Ⅲ~Ⅴ期,47例);再收集医院体检中心筛选出的与PD患者年龄和性别群体一致人群作为对照组(80例)。测定入组研究对象的血清Hcy、NFL水平,分析其在对照组和PD患者间差异及其在不同PD患者间变化情况,同时探究其对疾病及疾病严重程度的预测价值。结果PD患者血清Hcy、NFL水平高于健康体检人群(P<0.05)。PD患者血清Hcy、NFL水平按A、B、C依次递增,且组间比较差异有统计学意义(P<0.05),且A组血清Hcy、NFL水平低于B组和C组(P<0.05),B组血清Hcy、NFL水平低于C组(P<0.05)。PD患者早期组血清Hcy、NFL水平低于中晚期组(P<0.05)。Spearman相关性分析结果显示:血清Hcy、NFL与PD患者病程和疾病严重程度均呈正相关(P<0.05)。结论PD患者血清Hcy、NFL水平均相对较高,其水平随着疾病病程增加和严重程度增加呈上升趋势。
Objective To investigate the relationship between serum homocysteine(Hcy)and serum neurofilament light chain(NFL)and the disease duration and disease severity of Parkinson’s disease(PD).Methods A total of 80 patients with PD admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University from May 2018 to May 2020 were selected as the study subjects,and were divided into two subgroups based on their course of disease and disease severity.They were divided according to the course of disease:group A(course<5 years,44 cases),group B(5 years≤course<9 years,25 cases)and group C(course≥9 years,11 cases).The severity of the disease was evaluated using the Hoehn-Yahr grading scale,namely,the early stage group(stageⅠto stageⅡ,33 cases),the intermediate and late stage group(stageⅢto stageⅤ,47 cases).And then the hospital physical examination center screened a population consistent with the age and gender group of PD patients as control group(80 cases).Serum Hcy and NFL levels were measured in the enrolled subjects,and their differences between the control group and PD patients and their changes among PD patients were analyzed,and their predictive value for disease and disease severity was also investigated.Results The serum Hcy and NFL levels in PD patients were higher than those in control group(P<0.05).Serum Hcy and NFL levels in PD patients increased in the order of group A,group B and group C,and the differences between groups were statistically significant(P<0.05),and the serum Hcy and NFL levels in group A were lower than those in groups B and C(P<0.05),and the serum Hcy and NFL levels in group B were lower than those in group C(P<0.05),and the serum Hcy and NFL levels in early stage group were lower than those in late stage group(P<0.05).The results of Spearman correlation analysis showed that serum Hcy and NFL were positively correlated with disease duration and disease severity in PD patients(P<0.05).Conclusion The levels of serum Hcy and NFL in patients with PD are relatively high,with an upward trend as the disease duration and severity increase.
