摘要
目的观察中西医结合治疗亚急性特发性面神经麻痹患者的临床疗效。方法选取2018年12月—2020年12月期间明光市中医院收治的106例亚急性期特发性面神经麻痹患者,按照随机数字表法将其分为对照组与治疗组,每组各53例。对照组患者采用西医常规治疗,治疗组患者采用针刺联合西医常规治疗,取穴以头面部为主,采用较重刺激手法和电针。治疗14 d后,观察比较两组患者临床疗效,治疗前后House-Brackmann面神经功能评分、中医症状体征评分(额肌运动、眼睑开合、鼻唇沟深浅、耸鼻运动、口角歪斜、鼓腮漏气、食物滞留、颈阔肌收缩功能、味觉障碍、听觉过敏、流泪不适、下颌角或乳突疼痛)情况。结果治疗后治疗组总有效率95.92%(47/49)明显高于对照组75.00%(36/48),差异有统计学意义(P<0.05)。治疗后两组患者House-Brackmann分级评分均较治疗前明显降低,差异有统计学意义(P<0.05);且治疗组House-Brackmann分级评分均较对照组明显降低,差异有统计学意义(P<0.05)。治疗后两组患者中医症状体征评分均较治疗前明显降低,差异有统计学意义(P<0.05);且治疗组中医症状体征评分均较对照组明显降低,差异有统计学意义(P<0.05)。结论针刺联合西药治疗亚急性特发性面神经麻痹可有效抑制神经坏死变性,改善面肌的血流,促进面神经的修复。
Objective To observe the clinical effect of the combination of traditional Chinese and western medicine on the treatment of subacute idiopathic facial paralysis.Methods A total of 106 patients with subacute idiopathic facial paralysis who were admitted to Mingguang Hospital of Traditional Chinese Medicine from December 2018 to December 2020 were included and divided into a control group and a treatment group by the random number table method,with 53 cases in each group.The patients in the control group were treated with conventional western medicine,while the patients in the treatment group were treated with acupuncture combined with conventional western medicine.The acupuncture points were mainly on the head and face,and heavier stimulation and electric acupuncture were used.After treatment for 14 days,the clinical effects of the two groups were compared.The House-Brackmann facial nerve function scores and traditional Chinese medicine(TCM)syndrome scores(frontal muscle movement,opening and closing eyelid,nasolabial fold depth,nose shrugging movement,distortion of commissure,failed to check blowing,food stagnation in mouth,systolic function of platysma muscle,dysgeusia,hyperacusia,weeping,and pain in mandibular angle or mastoid process)of the two groups before and after the treatment were compared.Results After treatment,the total clinical effective rate of the treatment group was 95.92%(47/49),which was significantly higher than that of the control group[75%(36/48)](P<0.05).After treatment,the House-Brackmann grading scores of the two groups were lower than those before treatment(P<0.05),and the score of the treatment group was lower than that of the control group(P<0.05).After treatment,the TCM syndrome scores of the two groups were lower than those before treatment(P<0.05),and the score of the treatment group was lower than that of the control group(P<0.05).Conclusion Appropriate acupuncture combined with western medicine has a curative effect on subacute idiopathic facial paralysis,which effectively inhibits nerve necrosis and degeneration,improves the blood flow of facial muscle,and promotes the repair of the facial nerve.
