摘要
目的:探讨腹腔镜下切取带蒂大网膜重建非哺乳期乳腺炎(NPM)患者术后乳房部分缺损的效果。方法:26例NPM患者乳房行病变腺体切除后,18例腹腔镜下切取带蒂大网膜填充乳房缺损组织(观察组),8例行常规切除后对拢缝合(对照组)。比较两组手术时间、术中出血量、住院时间、术后乳房的美容效果及患者满意度。结果:观察组手术时间和住院时间均较对照组长(P<0.05),两组术中出血量和手术切除范围差异均无统计学意义(P>0.05)。术后随访6个月,观察组术后乳房美容效果及满意度均优于对照组(P<0.01)。结论:腹腔镜下切取带蒂大网膜能有效填充NPM术后乳房腺体组织缺损,美容效果好,无明显手术并发症,患者满意度高。
Objective:To observe the efficacy of laparoscopic resection of pedicled omentum in reconstructing partial breast defects in patients with non-puerperal mastitis.Methods:Twenty-six women with non-lactational mastitis underwent excision of the diseased glands, and then received reconstruction of the breast defect tissue by filling with laparoscopically resected pedicled omentum(n=18,observational group) or closed suture(n=8,control group) after conventional resection.The two groups were compared regarding operative time, intraoperative blood loss, length of hospital stay, postoperative breast cosmetic effect and patient satisfaction.Results:Operative time and postoperative hospital stay were longer in the observational group than in the control group(P<0.05),yet intraoperative blood loss and resected range of the lesion were not significantly different between groups(P>0.05).Postoperative follow-up for 6 months showed that patients in the observational group had better breast cosmetic results and higher overall satisfaction with the reconstruciton(P<0.01).Conclusion:Laparoscopic resection of pedicled omentum can effectively fill the defect of breast glandular tissue from non-lactation mastitis, and lead to favorable cosmetic effect, no obvious surgical complications, yet higher patient satisfaction.
