预先置入阻挡钉联合闭合复位髓内钉固定治疗股骨及胫骨骨折的疗效观察 被引量:3

Observation of efficacy of pre-placement of blocking screw combined with closed reduction and intramedullary nailing in the treatment of femoral and tibial fractures

摘要 目的探讨预先置入阻挡钉联合闭合复位髓内钉固定在股骨和胫骨骨折中的疗效。方法87例股骨和胫骨骨折患者,根据入院顺序分为对照组(44例)及观察组(43例)。对照组行闭合复位髓内钉固定治疗,观察组在对照组基础上预先置入阻挡钉。比较两组手术时间、术中出血量、住院时间、骨折愈合时间、冠状面成角、矢状面成角及临床疗效。结果观察组手术时间(73.21±8.06)min长于对照组的(71.85±7.69)min,术中出血量(135.36±32.28)ml多于对照组的(133.97±32.07)ml,但差异无统计学意义(P>0.05);观察组住院时间(13.64±3.14)d、骨折愈合时间(14.34±2.91)周短于对照组的(15.71±3.64)d、(16.68±3.23)周,冠状面成角(2.31±0.52)°和矢状面成角(1.37±0.34)°小于对照组的(3.14±0.73)、(2.30±0.45)°,差异有统计学意义(P<0.05)。观察组优良率86.05%高于对照组的68.18%,差异有统计学意义(P<0.05)。结论预先植入阻挡钉结合闭合复位髓内钉治疗股骨和胫骨骨折的疗效满意,能促进骨折愈合及患肢功能恢复。 Objective To discuss the efficacy of pre-placement of blocking screw combined with closed reduction and intramedullary nailing in the treatment of femoral and tibial fractures.Methods A total of 87 patients with femoral and tibial fractures were divided into control group(44 cases)and observation group(43 cases)according to the order of admission.The control group was treated with closed reduction and intramedullary nailing,and the observation group was pre-placed with blocking screw on the basis of the control group.The operation time,intraoperative blood loss,hospitalization time,fracture healing time,coronal plane angle,sagittal plane angle and clinical efficacy were compared between the two groups.Results The operation time(73.21±8.06)min of the observation group was longer than(71.85±7.69)min of the control group,and the intraoperative blood loss(135.36±32.28)ml was more than(133.97±32.07)ml of the control group,but the difference was not statistically significant(P>0.05).In the observation group,the hospitalization time(13.64±3.14)d,and the fracture healing time(14.34±2.91)weeks were shorter than(15.71±3.64)d and(16.68±3.23)weeks in the control group,and the coronal plane angle(2.31±0.52)°and sagittal plane angle(1.37±0.34)°were smaller than(3.14±0.73)and(2.30±0.45)°in the control group.All the differences were statistically significant(P<0.05).The excellent and good rate 86.05%of the observation group was higher than 68.18%of the control group,and the difference was statistically significant(P<0.05).Conclusion Pre-placement of blocking screw combined with closed reduction and intramedullary nailing in the treatment of femoral and tibial fractures is satisfactory,and can promote fracture healing and functional recovery of affected limbs.
