摘要
目的评价补脾益肺固本汤对上呼吸道感染患者免疫球蛋白含量变化的影响。方法选择90例上呼吸道感染患者随机分成研究组和对照组,每组45例。对照组患者给予莫西沙星治疗,研究组患者在给予莫西沙星的基础上,同时口服补脾益肺固本汤进行治疗。7 d后对比患者治疗前后的临床表现症状,即咳嗽消失时间、鼻塞消失时间、退热时间和肺部罗音消失时间;并统计中医证候积分变化评价两组治疗疗效。同时我们检测了两组患者治疗前后免疫相关指标[血清炎性因子白细胞介素6(IL-6)、白细胞介素8(IL-8)、肿瘤坏死因子(TNF-α)、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)、血清免疫球蛋白(IgG、IgA、IgM)含量]的变化。结果研究组治疗有效率为93.33%(42/45),高于对照组的77.78%(35/45);研究组的主要中医证候积分均低于对照组,差异有统计学意义(P<0.05);对比两组患者的咳嗽消失时间、鼻塞消失时间、退热时间和肺部罗音消失时间指标,相对于对照组,研究组显示出明显优势(P<0.05);这表明研究组的疗效优于对照组。在改善患者体内免疫指标上,研究组治疗后的血清炎性因子IL-6、IL-8、TNF-α、hs-CPR、PCT、免疫功能指标IgA和IgM含量均低于对照组,IgG高于对照组,数据差异有统计学意义(P<0.05)。结论补脾益肺固本汤结合莫西沙星治疗上呼吸道感染较单一使用莫西沙星疗效更优,能有效的缓解患者咳嗽、鼻塞、发热等临床症状,改善面色苍白、多汗、懒言及食欲下降等中医证候,提高治疗有效率。同时补脾益肺固本汤中含有的多味中药能有效降低体内炎症含量表达,提高患者免疫能力,值得临床推广应用。
Objective To evaluate the effect of Bupi Yifei Guben Decoction(补脾益肺固本汤)on the change of immunoglobulin level in patients with upper respiratory tract infection.Methods Ninety patients with upper respiratory tract infection were selected and divided into study group and control group according to randomized control method,with 45 cases in each group.The control group was treated with moxifloxacin,and the study group was treated with Bupi Yifei Guben Decoction on the basis of the treatment in the control group.Both groups were treated for 7 days.The two groups'traditional Chinese medicine symptom integral,curative effect,fever disappearing time,nasal congestion,cough disappearing time and pulmonary rale disappearing time were compared.Before and after treatment,the serum levels of inflammatory cytokines IL-6,IL-8,TNF-α,hs-CPR and PCT levels,and serum immunoglobulin(IgG,IgA,IgM)level of two groups were compared.Results The therapeutic effect of the study group was 93.33%(42/45),higher than that of the control group(77.78%,35/45).After treatment,the scores of TCM symptoms,such as pallor,hyperhidrosis,laziness and decreased appetite,were lower in the study group than those in the control group(P<0.05).The time of antipyretic,nasal congestion,cough and raloid in the study group was lower than that in the control group(P<0.05).The serum levels of inflammatory cytokines IL-6,IL-8,TNF-α,hs-CPR and PCT in the study group were lower than those in the control group(P<0.05).The levels of IgA and IgM of the immune function indexes after treatment in the study group were lower than those in the control group,and IgG levels were higher than those in the control group(P<0.05).Conclusion The combination of Bupi Yifei Guben Decoction with moxifloxacin in the treatment of upper respiratory tract infection has significant curative effect and rapid effect.It can effectively alleviate the clinical symptoms of patients,reduce the level of inflammation in vivo,improve the immune ability of patients and improve the therapeutic effect,which is worthy of clinical promotion and application.
