抗核抗体自动间接免疫荧光检测性能分析

Analysis of the performance of automatic indirect immunofluorescence detection for antinuclear antibodies

摘要 目的评估全自动间接免疫荧光分析仪对抗核抗体(ANA) HEP-2细胞荧光核型滴度与人工判读结果的一致性验证及临床的应用价值。方法通过helios自动判读系统与人工荧光显微镜判读,对本院2018年8月-2019年2月352例门诊及住院患者的ANA荧光载片判读,二者结果进行比对,验证HELIOS自动图形判读系统对抗核抗体荧光核型识别、终点滴度的预测准确性。结果 HELISO判读ANA荧光片阴性/阳性结果与人工判读结果的总符合率为98.29%,其阳性符合率为100.00%,阴性符合率为92.40%。ANA细胞滴度仪器与人工比对,两者符合率达96%。对ANA细胞的荧光核型识别,HELIOS荧光分析仪对复合核的ANA细胞判读能力上略偏弱外,总体核型识别比较满意,结果符合率为91.21%。结论 HELIOS荧光分析仪,自动判读ANA荧光结果和人工肉眼判读ANA结果具有一致性。HELIOS检测ANA的实验程序,更具有标准化、简便性、稳定性,提高临床实验室对AID疾病认识甄别和自体抗体实验室标准化建立具有一定价值。 Objective To evaluate the consistency and its clinical application of fluorescent nuclear titer of ANA HEP-2 cells with the results of manual interpretation.Methods The ANA fluorescence slides of 352 outpatients and inpatients in our hospital from August 2018 to February 2019 were interpreted by HELISO automatic image interpretation system and artificial fluorescence microscope.The comparison of the two results verified the accuracy of the helios automatic image interpretation system in identifying the fluorescent karyotype of anti-nuclear antibody and predicting the final spot degree.Results The total coincidence rate between HELISO interpretation ANA fluorescent tablets negative/positive results and manual interpretation results was 98.29%,respectively positive for 100.00%and negative for 92.40%.The coincidence rate of ANA cell titer instrument and artificial comparison was up to 96%.HELIOS fluorescence analyzer was slightly weak in ANA cell interpretation ability of complex nucleus,and the overall karyotype recognition was satisfactory,with the coincidence rate of 91.21%.Conclusion HELIOS fluorescence analyzer,automatic ANA fluorescence interpretation results and human eye ANA interpretation results are consistent.The HELIOS experimental procedure for ANA detection is more standardized,convenient and stable,which is of certain value in improving the recognition and screening of AID diseases in clinical laboratories and establishing the standardization of autoantibody laboratories.
