摘要
目的对比InterTan与PFNA治疗老年股骨粗隆间骨折临床疗效。方法将96例老年股骨粗隆间骨折患者随机分为InterTan组(n=50)和PFNA组(n=46)。InterTan组行InterTan内固定治疗,PFNA组行PFNA内固定治疗。观察2组手术相关指标术中出血、引流量、手术时间、早期下床时间、住院时间,手术相关并发症如肺部感染、尿路感染、切口感染、切口肿胀,术后恢复情况(负重功能锻炼时间、骨折愈合时间),骨折愈合相关并发症(延迟愈合、畸形愈合、内固定失败),骨折愈合优良率及髋关节功能。结果InterTan组术中出血、引流量多于PFNA组(P<0.05),手术时间多于对照组(P<0.05),早期下床时间早于对照组(P<0.05),住院时间少于对照组(P<0.05),差异均具有统计学意义。2组并发症主要为肺部感染、尿路感染、切口感染、切口肿胀,各并发症发生率及总发生率比较差异无统计学意义(P>0.05)。InterTan组负重功能锻炼时间、骨折愈合时间早于对照组(P<0.05),骨折愈合相关并发症发生率低于PFNA组(P<0.05)。InterTan组Harris评分高于PFNA组(P<0.05),骨折愈合优良率高于PFNA组(P<0.05)。结论InterTan与PFNA治疗老年股骨粗隆间骨折各有优缺点,临床应针对具体病例合理地选择使用。
Objective To compare the clinical efficacy of InterTan and PFNA in treatment of intertrochanteric fractures of the elderly.Methods A total of 96 elderly patients with intertrochanteric fractures were randomly divided into InterTan group(n=50)and PFNA group(n=46).InterTan group received InterTan internal fixation,and PFNA group received PFNA internal fixation.The operation-related indicators,including intraoperative bleeding,drainage,duration of operation,early off-bed time,and length of hospital stay,operation-related complications such as lung infection,urinary tract infection,incision infection and incision swelling,postoperative recovery(time of weight bearing exercise,fracture healing time),and complications related to fracture healing(delayed union,malunion,failure of internal fixation),good rate of fracture healing and hip joint function were observed and compared between the two groups.Results The intraoperative bleeding and drainage amount in InterTan group were significantly more than those in the PFNA group,the duration of operation was significantly longer than that in control group,the early off-bed time was significantly earlier than that in control group,and the length of hospital stay was significantly shorter than that in control group(P<0.05).Complications in the two groups mainly included pulmonary infection,urinary tract infection,incision infection,and incision swelling.There were no significant differences in the incidence rate of each complication and total incidence rate of complication(P>0.05).The time of weight-bearing exercise and fracture healing time in InterTan group were significantly earlier than those in control group(P<0.05),and the incidence rate of fracture healing-related complications was significantly lower than that in PFNA group(P<0.05).Moreover the Harris scores in InterTan group wer significantly higher than those in PFNA group(P<0.05),and the excellent and good rate of fracture healing was significantly higher than that in PFNA group(P<0.05).Conclusion InterTan and PFNA have their own advantages and disadvantages in the treatment of intertrochanteric fractures in the elderly,which,therefore,should be selected and used rationally in specific cases.
