温针灸联合独活寄生汤治疗膝骨性关节炎疗效及对关节滑液中骨桥蛋白和血管内皮生长因子水平影响 被引量:10

The curative effect of warm acupuncture combined with Duhuojisheng Decoction in the treatment of knee osteoarthritis and the impact on the osteopontin and vascular endothelial growth factor in the synovia fluid

摘要 目的探讨温针灸联合独活寄生汤对KOA疗效,及对关节滑液中OPN和VEGF水平影响。方法选择150例KOA患者,按分层随机化原则将患者随机分为A组、B组和C组,每组各50例;A组给予独活寄生汤治疗,B组给予温针灸治疗,C组温针灸联合独活寄生汤治疗;检测3组治疗前后关节滑液中OPN和VEGF浓度变化。结果 3组临床疗效比较,差异有统计学意义(χ2=12.90,P<0.05);治疗前和治疗第1周3组关节滑液中OPN和VEGF浓度差异无统计学意义(P>0.05);治疗第3周3组关节滑液中OPN和VEGF浓度差异有统计学意义(F值分别为6.95、9.96,P<0.01);关节滑液中OPN和VEGF浓度C组较A组和B组显著降低,差异有统计学意义(P<0.01)。结论温针灸联合独活寄生汤治疗膝骨性关节炎疗效显著,温针灸联合独活寄生汤能显著降低关节滑液中OPN和VEGF水平。 Objective To investigate the curative effect of warm acupuncture combined with Duhuojisheng Decoction in the treatment of knee osteoarthritis and the impact on the osteopontin( OPN) and vascular endothelial growth factor( VEGF) in the synovia fluid. Methods 150 patients with knee osteoarthritis were selected and randomly divided into group A,group B and group C according to the principle of stratified randomization,with 50 cases in each group; group A,B and C were respectively treated with Duhuojisheng Decoction,warm acupuncture and the combination of both. The levels of OPN and VEGF in the synovial fluid among the 3 groups were detected before and after treatment. Results The differences of clinical effects among the 3groups were statistically significant( χ2= 12. 90,P〈0. 05); there was no statistical significance on the difference of OPN and VEGF concentrations in the synovia fluid before and after 1 week's treatment among the 3 groups( P〉0. 05); in the third week of treatment,there was statistical significance on the differences in concentrations of OPN and VEGF in synovial fluid among the3 groups( F = 6. 95,F = 9. 96,P〈0. 01). The concentrations of OPN and VEGF in C group were significantly lower than that in group A and group B( P〈0. 01). Conclusion The curative effect of warm acupuncture combined with Duhuojisheng Decoction was significantly in the treatment of knee osteoarthritis,which can significantly decrease the concentrations of OPN and VEGF in synovia fluid.
