摘要
目的评价普通超声探头引导经皮经肝胆管穿刺置管引流术(PTCD)的临床价值。方法应用彩色超声诊断仪,普通3.5MHz探头,对73例梗阻性黄疸患者行经皮经肝胆管穿刺置管引流术。结果73例中一次性成功70例(95.9%),其中2例因引流管脱落而先后行再穿刺;3例首次穿刺失败,再次穿刺成功。62例为姑息性治疗,患者带管生存。11例行PTCD术后,黄疸消退再行外科手术治疗。结论PTCD术前减黄及对不能切除的晚期恶性肿瘤引起的阻塞性黄疸患者,既可以改善症状,又可延长生命。同时使用普通探头引导,使PTCD术更加灵活、准确、安全、实时,是固定不动的穿刺探头所不能比拟的。
Objective To evaluate clinical value of PTCD wi th common ultrasonic transducer.Methods Use color ultrasonic diagnostic instrument a nd common 3.5MHz transducer to make P TCD for 73cases of obstructive jaundice patients.Results Within the range of 73cases,the once-through cases accounted for 95.9%,and 2cases needed to re-make PTCD because the drainage tube was fallen off;3cases failed and re-make PTCD.The p alliative treatment was used in 62ca ses,so that the patients live with the tube on their bodies.After PTCD,jaundice in 11cases was disappeared,and then surg ery operation treatments were carried on for them.Conclusions To reduce jaundice symptom before op eration,and for the obstructive jau n-dice patients resulted from a malign ant tumor in advanced stage that can n ot be cut off.PTCD technique not only improve symp-toms ,but also prolong life.To drain with c ommon detector has flexible,correct and safe advantages,so it has much c linical value.[
