摘要
目的探讨激光辅助孵化(1aser-assisted hatching,LAH)对反复移植失败患者以及高龄患者冻融胚胎移植(frozen-thawed embryo transfer,FET)临床妊娠结局的影响。方法收集反复移植失败的患者98例,随机分为LAH 1组与对照组1,每组49例;高龄(≥38岁)FET患者90例,随机分为LAH 2组与对照组2,每组45例,LAH1组与LAH 2组患者均在移植前2小时进行LAH。结果 LAH1组的胚胎种植率、临床妊娠率和活产率均显著高于对照组1(28.84%vs11.5 3%;42.85%vs20.41%;36.7 3%vs16.32%);LAH 2组的胚胎种植率、临床妊娠率和活产率均高于对照组2(1 9.64%vs8.65%;42.22%vs20.00%;40.00%vs1 7.78%),差异有统计学意义(P<0.05)。结论 LAH可提高反复种植失败患者和高龄患者冻融胚胎移植的胚胎种植率、临床妊娠率和活产率,改善临床结局。
Objective: To determine whether laser-assisted hatching can improve clinical outcome of patients with recurrent implantation failure after in vitro fertilization and with advanced age in frozen-thawed embryos transfer. Methods: 98 patients with recurrent implantation failure were divided equally into LAH1 and controll groups.90 patients with advanced age were divided equally into LAH2 and control2 groups. On the day of embryo transfer, the zona pellucida of the selected embryos in the LAH groups were thinning by using laser system two hours before transfer. Results: In the patients with recurrent implantation failure, the rates of implantation, clinical pregnancy and live birth in LAH1 group were higher than which in control 1 group (28.84%vs11.53%; 42.85%vs20.41%; 36.73%vs16.32% respectively) . In the patients with advanced age, the rates of implantation clinical pregnancy and live birth in LAH2 group were higher than which in control 2 group (19.64% vs 8.65%; 42.22% vs 20.00%; 40.00% vs 17.78%) , the statistics had significant differences. Conclusion: The LAH can improve the rates of implantation clinical pregnancy and live birth witrecurrent implantation failure and advanced age and improve clinical outcomes.
