彩色多普勒超声在新生儿缺氧缺血性脑病早期诊断中的临床应用 被引量:10

摘要 目的探讨彩色多普勒超声在新生儿缺氧缺血性脑病(HIE)早期诊断中的临床价值。方法选取2012年1月-2014年9月在磁县人民医院妇产科出生并诊断为HIE的68例新生儿为研究对象,根据临床分度分为轻度组、中度组和重度组,同时选取40例健康新生儿分为健康组。对上述108例新生儿在其出生24、48、72 h时采用彩色多普勒超声对其脑实质回声变化情况和大脑中动脉的血流动力学参数Vs、Vd和RI进行动态监测,比较各组间上述指标的差异,同时与CT、MRI诊断结果进行比较。结果健康组新生儿24、48 h时的Vs、Vd高于3组患儿;3组患儿中,轻度组出生24、48 h时的Vs、Vd高于中度组和重度组,且中度组也高于重度组,各组之间结果比较差异均有统计学意义(P〈0.05)。健康组新生儿出生24、48 h时的RI低于3组患儿,轻度组出生24、48 h时的RI低于中度组和重度组,且中度组也低于重度组,各组之间结果比较差异均有统计学意义(P〈0.05)。4组新生儿出生72 h时的Vs、Vd、RI差异无统计学意义(P〉0.05)。68例患儿中8例轻度患儿超声结果未见显著异常,总诊断率为88.24%。结论新生儿MAC血流动力学多普勒超声结果可作为早期诊断HIE的重要依据。 Objective To explore clinical value of color Doppler ultrasound in early diagnosis of newborn hypoxic-ischemic encephalopathy. Methods 68 newborns born in department of obstetrics and gynecology of Ci County People′s Hospital and diagnosed as HIE from January 2012 to September 2014 were selected as study object.Patients were divided into slight group,median group and serve group according to clinical grading,and 40 healthy newborns were selected as health group.Color doppler ultrasound was used to dynamic monitor the change of brain parenchyma echo and hemodynamic parameters liked Vs,Vd and RI of cerebral middle artery of 108 newborns after 24 h,48 h and 72 h birth.The difference of that index among four groups was compared,and its diagnostic result was compared with CT,MRI. Results The index of Vs and Vd in healthy group at 24 h and 48 h birth was higher than that of illed children the three groups respectively;among illed children in three groups,the index of Vs and Vd in slight group at 24 h and 48 h birth was higher than that of median group and serve group respectively,while median group was higher than that of serve group,and the difference was statistical significance between the groups(P<0.05).The index of RI in health group at 24 h and48 h birth was lower than that of illed children in three groups,the index of RI in slight group at 24 h and 48 h birth was lower than that of median group and serve group,while median group was lower than that of serve group,and the difference was statistical significance between the groups(P<0.05).The index of Vs,Vd and RI had no statistical significance at 72 h birth among four groups(P>0.05).Among 68 illed patients,8 slight illed children did not see any obvious abnormity in the ultrasound results,and the total diagnosis rate was 88.24%. Conclusion Doppler ultrasound results of newborns MAC hemodynamics can be an important basis for early diagnosis of HIE.
