血浆RUNx3启动子甲基化对宫颈癌早期诊断的意义 被引量:4

摘要 目的探讨血浆RUNx3启动子甲基化作为预测宫颈早期癌变生物学标志的可能性。方法以2014年3月至2015年12月在我科就诊的40例宫颈癌患者为研究对象,按1:1个体配对的方式选择同期就诊的40例宫颈上皮内瘤样变和因其他原因切除正常宫颈组织的患者为对照,采用甲基化特异性聚合酶链反应(methylation specific PRC,MCP)方法检测正常宫颈组织、宫颈上皮内瘤样变组织、宫颈癌组织及其对应血浆RUNx3启动子甲基化状态,分析组织与血浆两者RUNx3启动子甲基化是否存在相关性。结果在正常宫颈组织标本中检出1例(2.5%,1/40)RUNx3启动子甲基化异常表达,在血浆中未检出在RUNx3启动子甲基化异常表达。宫颈上皮内瘤样变患者组织标本中检出13例(32.5%,13/40)RUNx3启动子甲基化异常表达,在血浆中检出在10例(25.0%,10/40)RUNx3启动子甲基化异常表达。宫颈癌患者组织标本中检出34例(85.0%,37/40)RUNx3启动子甲基化异常表达,在血浆中检出在29例(72.5%,29/40)RUNx3启动子甲基化异常表达。三组患者组织标本中RUNx3启动子甲基化异常表达发生率差异有统计学意义(P<0.05),血浆中RUNx3启动子甲基化异常表达发生率差异亦有统计学意义(P<0.05)。不同年龄、肿瘤分化程度、淋巴结转移、病理类型、分期、肿瘤大小宫颈癌患者组织及血浆中RUNx3启动子甲基化异常表达差异均无统计学意义(P>0.05)。组织及血浆RUNx3启动子甲基化异常表达呈显著的正相关(r=0.62)。结论血浆RUNx3启动子甲基化表现和组织RUNx3启动子甲基化表现具有一致性,血浆RUNx3启动子甲基化异常提示了宫颈癌的发生发展,有望作为宫颈癌早期预测的生物学标志。 bObjective Discuss the possibility of the methylation of promoter RUNx3 in plasma as a biomarker on predicting the early-stage cervical carcinoma. Methods The study enrolled 40 cervical carcinoma patients who were in treatment in the department of Central people’ s hospital from March 2014 to December 2015. 40 patients who had cervical intraepithelial neoplasias and 40 patients who had excision of cervical tissues for other reasons were selected in a 1:1 ratio matching the experimental group at baseline as the control groups. We detected the normal cervical tissues,cervical intraepithelial neoplasias tissues,cervical carcinoma tissues and their methylation of promoter RUNx3 by methylation specific PRC. And we analyze the relevance between the tissues and plasma RUNx3. Results 1 sample(2. 5%,1/40) in the normal cervical tissues with abnormal expression of promoter RUNx3 methylation was detected. No sample was detected in the plasma. 13 samples(32. 5%,13/40) in the cervical intraepithelial neoplasias tissues with abnormal expression of promoter RUNx3 methylation were detected. 10 samples ( 25. 0%,10/40 ) were detected in the plasma. 34 samples(85. 0%,37/40) in the cervical carcinoma tissues with abnormal expression of promoter RUNx3 methylation were detected. 29 samples(72. 5%,29/40) were detected in the plasma. The differences of the abnormal expression of promoter RUNx3 methylation among the three groups were significant(P〈0. 05) in both the tissues and the plasma. There was no significance(P〉0. 05) between the methylation in tissues and plasma when took the age,the degree of tumor differentiation,the lymphatic metastasis,the pathological pattern,the neoplasm staging and the size of the tumor into consideration. There was a significantly positive correlation ( r=0. 62 ) between the methylation in the tissues and the plasma. Conclusion The methylation of RUNx3 in plasma is in accordance with that in tissue. The methylation of RUNx3 in plasma reminders the development of cervical carcinoma and may become the biomarker in predicting early-stage cervical carcinoma.
