摘要
目的探讨股骨近端防旋髓内钉(PFNA)内固定微创治疗股骨粗隆间骨折(IFF)的临床效果。方法将2013年2月至2015年2月期间我院骨科收治的78例股骨粗隆间骨折患者按照数字表法随机分为两组,对照组39例给予动力髋螺钉系统(DHS)治疗;实验组39例采用微创PFNA内固定治疗,并比较两组患者的临床疗效情况。结果实验组的总优良率为87.18%显著的高于对照组的74.36%(χ2=2.0635,P<0.05),实验组手术时间、出血量、负重时间、骨折愈合时间均明显优于对照组(P<0.05),两组住院天数比较差异无统计学意义(P>0.05),实验组的总并发症发生率为5.13%与对照组的7.69%(χ2=0.259,P>0.05)。结论微创股骨近端防旋髓内钉内固定治疗股骨粗隆间骨折临床疗效较佳,够达到坚强的中心固定、良好的支撑,具有微创、手术时间短、出血少等优势,且术后并发症少,较为安全。
Objective To investigate the clinical effect of minimally invasive treatment of intertrochanteric fractures of femur (IFF) with proximal femoral nail (PFNA). Methods From February 2015 to February 2013, 78 cases of intertrochanteric fracture patients in our hospital were randomly divided into two groups, the control group was treated with dynamic hip screw system ( DHS), and the 39 cases in the observation group were treated with minimally invasive PFNA, compared the clinical efficacy of the two groups. Results The total yield of observation group was 87.18%, which was significantly higher than that of control group 74.36% (~2 = 2. 0635 ,P 〈 O. 05 ). The operation time, bleeding volume, weight bearing time, fracture healing time of observation group were significantly better than those in control group ( P 〈 0. 05 ), and the total complication rate in the observation group was 5. 13% which similar to 7.69% in control group ( ~2 = 0. 259, P 〉 0. 05 ). Conclusions Minimally invasive treatment of femoral intertrochanteric fractures with good clinical efficacy, it is strong enough to reach a strong central fixation, good support, with minimally invasive, short operation time, less bleeding, and less postoperative complications, and it is safe.
