摘要
目的观察异丙酚和异氟醚对海水浸泡后单肺通气(OLV)氧合的影响。方法健康犬20只随机分为两组,异丙酚组10只微泵输入剂量4mg·kg-1/h;异氟醚组10只持续吸入1.5%;建立胸部开放性海水浸泡犬模型。监测海水浸泡30min,双肺通气(TLV)30min,单肺通气30、60min等时点的股动脉血气,并计算心脏指数(CI),氧供指数(DO2I)、氧耗指数(VO2I)数值。结果海水浸泡30min时点表现为严重低氧血症、高碳酸血症,Qs/Qt增加,CI下降。两组双肺通气30minPaO2、PaCO2、DO2I、VO2I与海水浸泡30min时点比有显著性差异(P<0.05),两组单肺通气30、60min时点分别与双肺通气30min时点有显著性差异(P<0.05),单肺通气30min、60min两时点无显著性差异(P>0.05)。Qs/Qt、CI从双肺通气开始分别逐渐上升和下降,CI最低为(2.4±0.7)L·min-1/m2,存在心源性休克。结论异丙酚和异氟醚对胸部开放性海水浸泡后单肺通气氧合无影响。心源性休克表示预后差。
Objective To study the effect of Propofol and Isoflurane on OLV oxygenation in sea water soaked-lung.Methods Twenty dogs were divided into two groups,the model of sea water soaked-lung was set up;30minutes later,TLV30minutes and OLV30minutes,60minutes were performed respectively,then the count of blood gas in hip artery was taken.CI,DO 2 I and VO 2 I were calculated in different groups.Results After30minutes of sea water soaked,the dog's blood appears severe hypoxemia and hypercapnia,the value of Qs /Qt was increased and CI was decreased.Between TLV30minutes group and sea water soaked30minutes group,the PaO 2 ,PaCO 2 and VO 2 I had a significant difference(P<0.05).Between groups of OLV30minutes,60minutes and TLV30minutes,these values had also a significant difference(P<0.05),but there were no significant difference between OLV30minutes and OLV60minutes(P>0.05).Qs /Qt and CI was increased and decreased respectively from the TLV.The lowest CI is(2.4±0.7)L/min·m 2 .Cardiac shock happened.Conclusion Propofol and Isoflurane have no effect on OLV oxygenation in sea water soaked-lung.Cardiac shock can lead to worse results.
