摘要
目的 : 探讨带状疱疹并发脑膜脑炎的相关因素及发病机制。方法 : 汇集了山东省 12家省市级大医院皮肤科及神经内科住院患者的资料 ,共有 45例 ,从临床及实验室等方面进行了总结分析。结果 : 认为年龄不是主要因素 ,各种原因导致的免疫抑制是重要因素 ,颅神经尤其是三叉神经眼支受累引起脑炎占首位 ,其次是肋间神经。结论 : 带状疱疹并发脑膜脑炎的机制是由于VZV直接侵入脑组织 ,也存在免疫机制 ,因此在治疗中要合理的运用抗疱疹病毒药物、糖皮质激素及对症治疗 ,一般预后还是好的。
Objective:To determine the related factor and pathogenesis of herpes zoster complicated meningoencephalitis. Method: Forty-five inpatients from 12 large city hospitals of Shangdong Province were collected. The laboratory and clinical data were summarized and analyzed.Result: The results showed that immunosuppression caused by a variety of reasons was a key factor and the age of the patients was not correlated to the onset of meningoencephalitis in this group. Involvement of encephalic nerve, especially the eye branch of trigeminal nerve followed by intercostal nerve, was highly correlated to the meningoencephalitis.Conclusion: The mechanism of herpes zoster encephalitis might be caused by the invasion of verecella-zoster virus to brain tissue directly, and immune mechanism also plays a role. In the treatment of herpes zoster meningoencephalitis, if anti-virus drugs and steroid are used appropriately and as early as possible, the prognosis is usually good.
