摘要
目的 探讨诊断原发性肝癌的肝固有动脉血流动力学参数指标。方法 回顾性分析了 2 0例原发性肝癌的肝固有动脉内径及最大流速即峰流速 ,并与正常对照组及转移性肝癌组的肝动脉血流进行对照。结果 彩色多普勒血流显像 (CDFI)结果显示 :17例原发性肝癌其肝固有动脉内径 0 4~ 0 5cm、峰流速 80 0~ 16 0 0cm/s明显高于正常对照组及转移性肝癌组 ;肿块周边见彩色血流环绕、周边及其内亦可见较丰富的肝动脉血流信号。CDFI高度怀疑原发性肝癌的 3例患者 ,其肝固有动脉内径 0 2~ 0 .3cm ,未见明显增宽 ,但其峰流速均超过 40 0cm/s ,后确诊为原发性肝癌。结论 肝实性占位病变患者的肝固有动脉峰流速超过 80 0cm/s可作为诊断原发性肝癌的指标 ;肝固有动脉峰流速在40 0~ 80
Objective To explore the blood flow parameters of the proper hepatic artery to diagnose the hepatocellular carcinoma(HCC).Methods The diameter and maximum blood flow velocity(Vmax)of the proper hepatic artery in 20 cases of HCC were analysed and compared with the normal control group and the liver metastatic cancer(LMC)group.Results The result of CDFI:the diameter (0.4~0.5 cm) and Vmax (80.0~160.0 cm/s) of 17 cases of HCC were higher than normal control and LMC groups.The masses were circled by color blood flow,and much abundant blood flow signals of hepatic artery were observed in HCC in and around tumor.The diameter of proper hepatic artery of 3 cases of HCC doubted by CDFI didn't widen,but the Vmax > 40.0 cm/s, these cases were diagnosed as HCC later.Conclusion The Vmax of proper hepatic artery higher than 80.0 cm/s is an index to diagnose HCC;The Vmax of proper hepatic artery between 40.0-80.0 cm/s is highey doubtful of HCC.
