摘要
目的 通过检测烧伤病人心肌肌钙蛋白Ⅰ (cTnⅠ )的变化 ,了解细胞因子与脏器损伤的关系。方法 烧伤病人 2 8例 ,根据血清cTnⅠ值的高低分为A、B两组 ,分别于伤后 1、3、5、7、10天抽静脉血送检。cTnⅠ检测采用抗人心肌cTnⅠ单抗法 ,细胞因子检测采用双抗体夹心 (ELISA)法测定。结果 2 8例烧伤病人中有 11例cTnⅠ值高于正常值 ,17例cTnⅠ值在正常范围。两组之间差异有显著性 (P <0 0 5 )。A组与B组的IL 1浓度均高于正常值 3~ 7倍 ,A、B两组之间差异无显著性 (P >0 0 5 )。A组的IL 2浓度远高于B组 ,差异有显著性 (P <0 0 0 1)。A组的IL 6浓度早期时低于B组 ,差异有显著性 (P <0 0 5 )。A组的TNF浓度高于B组 ,差异有显著性 (P <0 0 5 )。结论 烧伤病人cTnⅠ的升高可能与IL 2、TNF的升高有关 ,提示烧伤后IL 2、TNF的升高与脏器损伤有密切关系。
Objective To investigatie the correlation between cardiac troponin Ⅰ and cytokines,which may play a potential role in mediating multiple organ damage after burns.Methods Twenty eight patients with burns covering 20 percent or more of TBSA were divided into two groups:A group with increased cTnⅠ and B group with normal cTnⅠ. Serial blood samples were collected at 1,3,5,7,10 days after the burn.We established a double monoclonal Sandwich enzyme immunoassay to measure cTnⅠ and ELISA to measure cytokines.Results Levels of serum cTnⅠ showed significant difference between the two groups( P <0 05).No significant difference was found in the levels of serum IL 1( P >0 05).Levels of serum IL 2 showed significant difference between the two groups( P <0 001).Levels of serum IL 6 showed significant difference between the two groups( P <0 05).Levels of serum TNF had significant difference between the two groups( P <0 05).Conclusions The increased formation of cTnⅠ may have a relation to some cytokines.It is worthy to explore the relation between cytokines and multiple organ damage after burns.
