血清降钙素原在呼吸机相关性肺炎中的临床诊断价值 被引量:1

摘要 目的探讨血清降钙素原(PCT)在呼吸机相关性肺炎(VAP)诊断中的临床应用价值。方法对48例VAP确诊患者48 h内行PCT、C反应蛋白(CRP)、白细胞计数(WBC)、中性粒细胞百分比(N)及体温(T)测定。同时对60例非VAP患者和20例健康体检者进行相关指标测定。结果 VAP组PCT高于非VAP组及正常对照组,差异有统计学意义(P<0.01),非VAP组与正常对照组间差异无统计学意义(P>0.05);PCT诊断VAP的敏感性为83.3%,特异性为88.3%。结论血清PCT在VAP的诊断中有较高的敏感性和特异性,优于传统的炎性指标,及时监测血清PCT可能有助于VAP的早期诊断和治疗。 Objective To investigate the serum procalcitonin (PCT) in ventilator-associated pneumonia (VAP) in the diagnosis of clinical value. Methods In 48 cases of VAP patients diagnosed by PCT within 48 hours, C reaction protein (CRP), white blood cell count (WBC), the percentage of neutrophils (N) and body temperature (T) determination. At the same time, 60 cases of non VAP patients and 20 healthy subjects related indexes determination. Results In VAP group, PCT was higher than that of VAP group and normal control group, the difference was statistically significant (P〈0.01), without statistical significance in non VAP group and normal control group (P〉0.05);the difference between the sensitivity of PCT diagnosis of VAP was 83.3%, the specificity was 88.3%. Conclusion Erum PCT has high sensitivity and specificity in the diagnosis of VAP, inflammatory index is better than the traditional, timely monitoring of early diagnosis and treatment of serum PCT may contribute to VAP.
