摘要
目的:研究单肺麻醉时对肺内分流(Q·s/Q·t)影响的规律性。方法:选择14例择期开胸手术病人,用静吸复合全麻,分别于麻醉前、双肺通气(TLV)15min、单肺通气(OLV)5、15、30、60min抽动脉血和混合静脉血行血气分析,计算Q·s/Q·t。结果:Q·s/Q·t在TLV时大于麻醉前(P<0.01),在OLV5min又大于TLV时(P<0.01),但与OLV60min相比无显著性差异(P>0.05)。PaO2在OLV15min低于双肺通气(P<0.01),但与OLV60min相比无显著性差异(P>0.05)。结论:在静吸复合麻醉下,Q·s/Q·t在OLV5min时最大,OLV60min尚未降低。PaO2在OLV15min时降至最低。
Objective:To study the change of intrapulmonary shunt (Q·s/Qs/Q·t) during one lung anesthesia Method:Fourteen patients undergoings elective openthoracic operation were chosen,anesthetized with enflurance combined intravenous anesthetics The mixed venous and arterial blood sample were collected for blood gases analysis before anesthesia and 15 min following two lung ventilation (TLV) and 5,15,30,60 min following OLV and Q·s/Qs/Q·t was calculated Results:Q·s/Qs/Q·t was higher at 5 min during OLV than that at TLV (P<0 01) Qt was higher at 5 min during OLV than that at TLV (P<0 01) Q·s/Qs/Q·t at 5 min was not significatly clifferent from at 60 min during OLV (P>0 05),PaO 2 was lower at 15 min value,but not significantly different from at 60 min during OLV (P>0 05) Conclusions:Anesthetized with enflurance in combination of intravenous anesthetics,Q·s/Qs/Q·t was maximal at 5 min and not decreased at 60 min during OLV PaO 2 was minimal at 15 min and not increased at 60 min during OLV
