摘要
目的探讨超敏C反应蛋白(hs-CRP)与冠心病患者冠状动脉病变程度及预后的关系。方法选择经冠脉造影证实的80例冠心病住院患者为研究对象,其中36例为稳定型心绞痛患者,44例为急性冠脉综合征患者。患者出院后随访3个月,入院期间检测患者血清hs-CRP、白细胞介素-6(IL-6)及肌钙蛋白-Ⅰ(CTn-Ⅰ)水平,采用Gensini积分评估冠脉病变程度。结果与稳定型心绞痛患者相比,急性冠脉综合征患者血清hs-CRP、IL-6及CTn-Ⅰ水平显著升高(P<0.05);患者住院及随访期间共有19例(23.8%)发生心血管事件,发生心血管事件者血清hs-CRP、IL-6及CTn-Ⅰ水平显著高于未发生心血管事件者(P<0.05);随着Gensini积分的增加,血清hs-CRP、IL-6及CTn-Ⅰ水平显著升高(P<0.05);Pearson相关分析结果显示,hs-CRP、IL-6及CTn-Ⅰ水平与Gensini积分显著正相关。结论 hs-CRP是冠心病患者预后的风险因素,且与冠脉病变程度显著相关。hs-CRP与IL-6、CTn-Ⅰ联合检测有利于评估患者病情及预后。
Objective To explore the relationship of hs-CRP with severity of coronary atherosclerosis and prognosis in coronary heart disease. Methods Eighty patients with coronary heart disease performed by coronary angiography were selected as research subjects, including 36 of stable angina pectoris and 44 of acute coronary syndrome. The patients were followed up for 3 months after discharge. Serum hs-CRP, IL-6 and CTn- Ⅰ were measured and G-ensini scores were used to evaluate the severity of coronary atherosclerosis. Results Compared with stable angina pa- tients, serum hs-CRP, IL-6 and CTn- Ⅰ in acute coronary syndrome patients greatly increased （P 〈 0.05）. Cardiovascular events occurred to 19 patients （23.8%） during their hospitalization and followed-up. Serum hs-CRP, IL-6 and CTn- Ⅰ in patients with cardiovascular events were much higher than those in patients without cardiovascular events（P 〈 0.05）. With the patients＇ Gensini scores increasing, serum hs-CRP, IL-6 and CTn- Ⅰ gradually increased （P 〈 0.05）. Pearson correlation analysis showed that hs-CRP, IL-6 and CTn- Ⅰ has positive correlation with Gensini scores. Conclusion Hs-CRP is the risk factor of prognosis for coronary heart disease patients, and has close relationship with severity of coronary atherosclerosis. Combinational detection of hs-CRP, IL-6 and CTn- Ⅰ is conducive to evaluating the disease and prognosis.
