摘要
目的观察总结锁定加压钢板治疗C型股骨远端骨折的临床应用和治疗效果,探讨C型股骨远端骨折的有效治疗方法。方法选择本院2010年1月～2012年1月的住院患者,均确诊为股骨远端骨折,AO分型均为C型。所有患者符合本研究的病例纳入标准,共42例。按照患者手术时间随机分为对照组20例和观察组22例,对照组采用髁部支持钢板固定术,观察组采用锁定加压钢板治疗,观察记录两组患者的手术时间、术中出血量和骨折的愈合时间等,并在术后第3、6、12个月进行随访,进行膝关节HSS评分和疗效观察。结果观察组患者在手术出血量、手术时间、骨折愈合时间与对照组比较,差异有统计学意义(P<0.05)。术后第3、6、12个月随访膝关节HSS评分比较,观察组患者膝关节HSS评分均高于对照组,差异有统计学意义(P<0.05)。结论锁定加压钢板治疗C型股骨远端骨折疗效较好,可减少术中创伤,减少并发症发生,促进术后患者膝关节功能恢复。
Objective To observe and summarize the clinical application and therapeutic effect of locking compression plate in treatment of distal femur fracture type C,and to explore effective method for the treatment of distal femur fracture type C. Methods Patients in our hospital during January 2010 to January 2012,who were diagnosed as distal femoral fracture with AO classification for type C. All the 42 patients conform to inclusive criteria of this study.According to the operation time,they were randomly divided into control group with 20 cases and observation group with 22 cases. Patients in control group were given ankle fixation with supportive plate,and patients in observation group were given locking compression plate.The operation time,intraoperative blood loss and fracture healing time in two groups were observed and recorded.And they were followed up and adopted knee HSS score and clinical observation at the third,sixth and twelfth month postoperatively. Results The intraoperatve blood loss,operation time and fracture healing time of observation group compared with that of control group,the difference was statistically significant （ P＆lt;0.05）. Comparison of knee HSS score at the third,sixth and twelfth month postoperatively,knee HSS score of patients in observation group was higher than that of patients in control group,curative effect in observation group was better,and the difference was statistically significant （P＆lt;0.05）. Conclusion Locking compression plate has a good curative effect in treatment of distal femur fracture type C.It can reduce intraoperative trauma and complications.And it can promote the recovery of knee joint function after operation.
