^(131)Ⅰ联合左旋甲状腺素对分化型甲状腺癌术后患者骨代谢的影响 被引量:3

摘要 目的探讨131Ⅰ联合左旋甲状腺素对分化型甲状腺癌(DTC)术后患者骨代谢的影响。方法 80例DTC患者随机分为抑制组与替代组,每组40例,均实施双侧甲状腺及峡部切除术,术后均给予131I+优甲乐治疗,抑制组优甲乐给予抑制剂量(2.5μg/kg),替代组优甲乐给予替代剂量(2.0μg/kg),同时选取40例健康人作为对照组,对比三组甲功及骨代谢情况。结果抑制组FT3、FT4均显著高于替代组与对照组(P<0.01),且TSH显著低于替代组与对照组(P<0.01);三组血清钙、磷、碱性磷酸酶及骨密度比较差异无统计学意义(P>0.05)。结论 DTC患者术后131I+左旋甲状腺素治疗对骨代谢影响较小。 Objective To study the effects of combination of ^131Ⅰ and levothyrocine on bone metabolism in patients with differentiated thyroid carcinoma(DTC) after surgery. Methods 80 patients with DTC were randomly divided into suppression group (n = 40 )and alternative group (n = 40 ) , all the patients were treated with bilateral thyroid and isthmus resection and ^131Ⅰ+ levothyrocine after surgery, suppressive dose (2. 5 μg/kg)of levothyrocine was given to suppression group and replacement dose( 2. 0 μg/kg)to alternative group. 40 healthy subjects were selected as the control group, then thyroid function and bone metabolism of the three groups were compared. Results FI3 and FT4 were significantly higher in suppression group than those in alternative group( P 〈 0. 01 ) , but TSH was significantly lower in suppression group( P 〈 0. 01 ) ;There was no significantly difference on serum calcium, phosphorus, alkaline phospha- tase and BMD among the three groups ( P 〉 0. 05 ). Conclusion The postoperative ^131Ⅰ + levothyroxine therapy has lit- tie effect on bone metabolism in patients with DTC.
