CXCL13及其受体CXCR5表达与非小细胞肺癌淋巴结转移的关系 被引量:10

摘要 目的初步探讨趋化因子CXCL13及其受体CXCR5在非小细胞肺癌及其转移淋巴结中的表达情况,并研究其与淋巴结转移之间的关系和意义。方法选取肺癌组织60份和转移的淋巴结标本30份,同时选取正常肺组织30份以及无转移的淋巴结组织30份,利用免疫组化检测CXCL13、CXCR5在不同组织中的表达情况。结果①部分肺癌组织周边的毛细血管后微静脉存在向类高内皮微静脉转化,内皮高大,胞质丰富,突起增多。CXCL13主要表达在肿瘤细胞、间质浸润的淋巴细胞以及淋巴结的树突细胞,CXCR5主要表达在肺癌组织周边的毛细血管后微静脉内皮细胞、淋巴结滤泡间区的高内皮后微静脉内皮细胞。CXCL13、CXCR5在癌组织中表达高于正常肺组织(P均<0.05);在转移淋巴结中的表达高于无转移淋巴结(P均<0.05)。②CXCL13及其受体CXCR5表达在正常肺组织、无转移淋巴结、肺癌组织以及转移淋巴结中均存在密切相关(r=0.569 1,P=0.002;r=0.527 1,P=0.001;r=0.704 4,P=0.005;r=0.767 1,P=0.002)。③CXCL13的阳性表达水平(即灰度单位)与淋巴结转移、脉管内癌栓、细胞分化程度之间存在密切相关(P均<0.05),与肿瘤体积、肿瘤分期、侵犯肺膜无关(P均>0.05);CXCR5的阳性表达水平与淋巴结转移、脉管内癌栓、肿瘤体积存在明显相关(P均<0.05),与细胞分化、侵犯肺膜、肿瘤分期无明显相关(P均>0.05)。结论 CXCL13及CXCR5表达参与正常肺组织的淋巴细胞归巢和肺癌组织的淋巴结转移,两者在肺癌的淋巴结转移中发挥协同作用,肿瘤周边内皮细胞的高内皮化是远处转移的结构基础。
