P63TTF-1及CD56联合相关免疫组化在肺癌诊断中的意义 被引量:9

摘要 目的探讨P63、TTF-1及CD56联合相关免疫组化在肺癌诊断中的价值。方法对180例已诊断为肺癌的标本行免疫组化染色,所选抗体为:P63、CK5/6、CK18、CEA、TTF-1、CD56。结果 P63、CK5/6诊断鳞状细胞癌的灵敏度分别为:93.4%(71/76)、87.3%(62/76),特异度分别为:92.3%(96/104)、87.5%(91/104);P63联合CK5/6诊断鳞状细胞癌的灵敏度和特异度分别为:97.4%(74/76)、84.6%(88/104)。CK18、CEA、TTF-1诊断腺癌方面的灵敏度分别为:98.9%(91/92)、87.0%(80/92),特异度分别为:54.5%(48/88)、85.2%(75/88)、90.0%(79/88);CK18、CEA、TTF-1联合诊断腺癌方面的灵敏度和特异度分别为100%(92/92)和54.5%(48/88),其特异度明显下降。CD56在诊断肺小细胞肺癌方面的灵敏度和特异度分别为100%(12/12)和99.4(167/168)。结论 P63、CK5/6、CEA、TTF-1及CD56抗体联合应用对肺癌的诊断及组织学分型具有较好的效果。 Objective To investigate the Diagnostic value of the combined Immunohistochemistry of P63, TTF-1, CD56 and related markers in lung cancer. Methods The expression of P63,CKS/6,CK18, CEA,TTF-1 and CD56 in 180 cases of lung cancer specimens were detected by immunohistochemistry. Results 1)Diagnostic Sensitivity of Squamous cell carcinoma using measurement of P63 and CK5/6 Immunohistochemistry, were 93. 4% (71/76) and 87.3 (62/76) respectively and Specificity were 92. 3 % (96/104) and 87. 5 % (91/104) respectively. Sensitivity and specificity of the combination of P63 and CK5/6 were respectively 97.4o//oo (74/76) and 84.6% (88/104). 2) With respect to Adenocarcinoma, Sensitivity of CK18, CEA and TTF-1 were respectively 98. 90/6 (91/92) ,87.0% (80/92) and 93.5% (86/92) ; Specificity were respectively 54.5% (48/88), 85.2% (75/88) and 90.0% (79/88) ; Sensitivity and specificity of the combination of CK18,CEA and TTF-1 were respectiveIy 100% (92/92) and 54.5% (48/88) ,which decreased seriously. 3) With respect to small cell lung cancer, Sensitivity and specificity of CD56 were respectively 100% (12/12) and 99. 4(167/168). Conclusion The combined Immunohistochemiztry of P63, CKS/6,CEA,TTF-1 and CD56 performed effectively in the diagnose and clarifying Histological type of lung cancer.
