HPV16/18感染、p53蛋白、PCNA表达与维吾尔族子宫颈鳞癌发病关系的研究

摘要 探讨新疆维吾尔族子宫颈鳞癌发生中HPV16/18、p53、PCNA三因素的作用及相关性。方法:应用PCR和免疫组化法检测127例维吾尔族患者宫颈组织的HPV16/18DNA和p53蛋白、PCNA的表达。结果:HPV16/18在正常宫颈、宫颈上皮内瘤样病变(CIN)、宫颈鳞癌中检出率分别为12.5%、22.5%和50.0%;p53、PCNA在三组阳性表达率分别为6.3%、22.5%、54.7%和6.3%、62.8%、81.2%。统计学分析表明,p53、PCNA在子宫颈癌与非癌组织中有显著性差异(P<0.01)。 p53蛋白、 PCNA表达与宫颈鳞癌临床分期、组织学分级、肿瘤大小无关(P>0.05)。有淋巴结转移的5例患者,3例为HPV16/18、p53、PCNA三项均阳性。结论:HPV16/18、p53、PCNA与子宫颈癌发生均有密切关系,可以作为子宫颈癌早期诊断及判断预后的分子生物学指标。 Objective: To investigate the relationship between human papillomavirus type 16/18 infection and the expressed products of tumor supressor gene p53 and proliferating cell nuclear antigen(PCNA). Methods: 127 paraffin-embedded specimens of cervix were examined including 64 cases of squamous carcinoma (SCC), 31 cases of cervical intraepithelial neoplasia (CIN) and 32 normal persons. The presence of HPV16/18 was detected by PCR technique, using immunostaining (LSAB) for p53 protein and PCNA overexpression.Results: Frequency of positive HPV16/18 DNA was 50.0% in SCC, 22.5% in CIN and 12.5% in normal controls. p53 expression was 54.7% in SCC, 22.5% in CIN and 6.3% in normal controls The PCNA expressive rates were 81.2%, 62.8%, 6.3% in SCC. CIN and normal cervix respectively. There was a significant difference between SCC and non-neoplastic lesions .Expressions of p53 protein and PCNA were not correlated to the grade, the tumor size, and the lymph node metastasis.Conclusion: ①HPV16/18 infection in Uygur women may have important ehologic significance of cervical carcinogenesis; ②Mutation of p53 tumor suppressor gene and expression of PCNA may have critical role on cervical carcinogenesis: ③There are close association of HPV16/18 infection and over-expression of P53 protein, PCNA with Uygur women cervical SCC could be as a valuable index for early diagnosis, and progonosis of cervical cancer.
