摘要
目的 探讨免疫印迹法在不同自身免疫性表皮下大疱病 (SABD)患者基底膜带自身抗体 (BMZ抗体 )分布情况及其在SABD诊断和鉴别诊断中的应用价值。方法 以热分离法制备的人真皮、表皮提取物作为抗原 ,免疫印迹法检测 97例SABD患者血清中BMZ抗体与基底膜抗原结合情况。结果 不同SABD患者血清中存在结合不同真皮、表皮抗原的IgG型和 (或 )IgA型BMZ抗体。大疱性类天疱疮患者血清中IgG和 (或 )IgA型BMZ抗体主要与相对分子质量为 2 30 0 0 0、180 0 0 0、16 5 0 0 0、130 0 0 0、115 0 0 0、970 0 0的表皮多肽结合 ;线性IgA大疱病患者血清中以结合 180 0 0 0、115 0 0 0、970 0 0表皮多肽的IgA型BMZ抗体为主 ;获得性大疱性表皮松解症患者血清中IgG型和IgA型BMZ抗体特异性结合真皮提取物中 2 90 0 0 0多肽 ;大疱性系统性红斑狼疮患者血清中以结合 2 90 0 0 0真皮多肽和16 5 0 0 0、130 0 0 0、970 0 0表皮多肽的IgG型BMZ抗体为主。结论 不同SABD患者血清中BMZ抗体存在异质性 ,免疫印迹法检测表皮下大疱病BMZ抗体阳性率较高 ,可用于该类疾病的辅助诊断和鉴别诊断。
Objectives To detect basement membrane zone autoantibodies (BMZ Ab) in sera of subepidermal autoimmune blistering diseases (SABD) and evaluate the value of immunoblotting in the diagnosis of various SABD. Methods We examined BMZ Ab in sera from 97 patients with SABD by immunoblotting on BMZ antigens from heat separated human dermis and epidermis. Results There were IgG and/or IgA BMZ Ab reacted with different BMZ antigens in different SABD. IgG and/or IgA BMZ Ab reacted with 230 000, 180 000, 165 000, 130 000, 115 000, 97 000 epidermal peptides were the main BMZ Ab in bullous pemphigoid sera. Linear IgA bullous dermatosis sera mainly consisted of IgA BMZ Ab reacted with 180 000, 115 000, 97 000 epidermal peptides. IgG and IgA BMZ Ab in epidermolysis bullosa acquisita sera specifically reacted with 290 000 dermal peptide. Bullous systemic lupus erythematosus sera mainly consisted of IgG BMZ Ab reacted with 290 000 dermal peptide and 165 000, 130 000, 97 000 epidermal peptides.Conclusion Our results suggested heterogeneity of SABD BMZ Ab. We considered that immunobloting detection of SABD BMZ Ab could be used for auxiliary diagnosis of SABD.
