摘要
目的通过临床观察确定"热敷药"治疗局限性硬皮病的临床疗效。方法将62例局限性硬皮病患者随机分为两组,治疗组32例,对照组30例。治疗组给予加热后"热敷药"外敷患处,对照组外用肝素钠软膏,两组均口服积雪苷片、VitE。连续用药6个月,比较治疗前后两组患者皮肤硬度积分、关节痛积分、关节功能积分,评估两组患者中医证候疗效及安全性。结果治疗组皮肤硬化积分、关节痛积分、关节功能积分优于对照组,差异有统计学意义(P<0.05);中医证候有效率比较治疗组痊愈5例,显效18例,有效6例,无效3例,总有效率71.86%;对照组痊愈3例,显效13例,有效9例,无效5例,总有效率53.33%,两组比较差异有统计学意义(P<0.01);安全性评估,治疗组1例患者轻度烫伤。结论 "热敷药"具有明显改善局限性硬皮病患者皮肤硬化的作用,且相对安全,值得在临床推广。
Objective To determine the clinical efficacy of the hot compress medicine on localized scleroderma through the clinical observation.Methods 62 cases of localized scleroderma were randomized into two groups,a treatment group（32 cases）and a control group（30 cases）.In the treatment group,the hot compress medicine was used locally.In the control group,heparin sodium cream was applied externally.Centella triterpenes tablets were taken orally,24 mg each time,three times a day and VitE,0.1 g each time,three times a day in either group.In 6 months of treatment,the skin sclerosis score,joint pain score and joint function score were compared before and after treatment for the patients between two groups.The efficacy on Chinese medicine symptoms and the safety were evaluated between two groups.Results In the treatment groups,the scores of skin sclerosis,joint pain and joint function were superior to those in the control groups,indicating statistical significant differences（P0.05）.Concerning to the comparison of the effective rate of Chinese medicine symptoms,in the treatment group,5 cases were cured,18 cases markedly effective,6 cases effective,3 cases failed and the total effective rate was 71.86%.In the control group,3 cases were cured,13 cases markedly effective,9 cases effective,5 cases failed and the total effective rate was 53.33%.The difference was significant statistically between two groups（P0.01）.For the safety evaluation,in the treatment group,1 case was mildly burned.Conclusion The hot compress medicine improves obviously in the skin sclerosis for the patients with localized scleroderma.It is relatively safe and deserves to be promoted in clinic.
