摘要
目的对比经输尿管镜肾囊肿内切开引流术和后腹腔镜肾囊肿去顶术临床疗效。方法选择54例肾囊肿患者随机分为观察组(经输尿管镜肾囊肿内切开引流术)28例和对照组(后腹腔镜肾囊肿去顶术)26例,分别使用经输尿管镜囊肿切开引流术和后腹腔镜肾囊肿去顶术治疗。观察和对比两组的临床疗效,并发症和随访结果。结果观察组中有3例进镜失败改后腹腔镜手术,25例患者均1次手术成功;对照组患者手术均1次成功,无中转开放手术病例。术后复查B超均提示肾囊肿消失或直径缩小至术前1/2以上。两组术中均未出现大出血、周围脏器损伤等严重并发症。两组患者下床活动时间比较差异无显著性(P>0.05),而在手术时间,术中出血量、患者术后服用止痛药、住院时间和住院总费用方面,观察组均优于对照组,差异均有显著性(P<0.05)。两组平均随访(8.4±3.6)个月。观察组肾囊肿复发有2例,对照组4例,差异无显著性(P>0.05)。结论经尿道输尿管镜技术处理部分肾囊肿具有近期疗效确切、微创、安全等优点,但远期疗效有待进一步观察。
Objective To compare the effect of urethroscopic incision and drainage and retroperitoneoscopic unroofing in treatment of renal cyst.Methods 54 patients of renal cyst were divided into observation group （urethroscopic incision and drainage,n=28） and control group （retroperitoneoscopic unroofing,n=26）.The clinical efficacy,complications and follow-up results were recorded and compared.Results 3 patients in observation group were transfered to retroperitoneoscopic treatment due to failed urethroscopy,while the remained 25 cases were successful.All the patients were successful in control group.The B-ultrasonic test after operation suggested the renal cyst disappeared or reduced to half the size of that preoperative.No severe complications such as massive hemorrhage,injury of surrounding organs occurred in both groups.The time of early ambulation had no significant difference between two groups （P0.05）,but time of operation,volume of intraoperative hemorrhage,cases of using anodyne,hospital stays and total cost in hospital in observation group were much better than those in control group （all P0.05）.The mean follow-up time was （8.4±3.6） months.The recurrence of renal cyst in two groups had no significant difference with 2 cases recurred in observation group and 4 in control group （P0.05）.Conclusion Urethroscopic incision in treatment of part renal cyst is featured by short-term efficacy,minimal invasion and safety,but the long-time efficacy needs a further study.
