地塞米松对特发性血小板减少性紫癜树突状细胞免疫活性的影响 被引量:2

摘要 目的:研究地塞米松对特发性血小板减少性紫癜(ITP)树突状细胞(DC)免疫活性的影响。方法:36例初诊ITP患者口服地塞米松40 mg/d,连用4 d;分离21例治疗有效患者治疗前后及正常对照外周血单个核细胞,加入细胞因子rhGM-CSF及rhIL-4培养诱导DC,用流式细胞技术检测DC表型;酶联免疫吸附法检测DC培养上清液中IL-12 p70。结果:①ITP患者DC的CD80和CD86阳性表达率分别为(51.60±13.47)%和(61.50±15.93)%,明显高于正常对照的(36.03±15.43)%和(40.28±11.49)%(P<0.05);治疗后ITP患者CD80和CD86表达率分别降为(36.48±15.19)%和(44.05±17.70)%,与治疗前相比,均差异有统计学意义(均P<0.05),但与对照组比较,差异无统计学意义(P>0.05)。②治疗前ITP患者组IL-12 p70为(67.52±14.43)pg/ml,明显高于正常对照的(39.78±10.03)pg/ml(P<0.05);治疗后ITP组IL-12 p70的表达降至(43.90±8.49)pg/ml,与治疗前相比差异有统计学意义(P<0.05),与正常对照相比差异无统计学意义(P>0.05)。结论:DC及其分泌的细胞因子在ITP发病中起重要作用,地塞米松可能通过抑制DC的免疫活性而达到治疗目的。 Objective:To study the effect of dexamethasone(DXM) on the immunoreactivity of dendritic cells in the patients with chronic idiopathic thrombocytopenic purpura.Method:Thirty-six newly diagnosed patients with chronic idiopathic thromobocytopenic purpura received oral high dose(HD) DXM at single daily doses of 40 mg for 4 consecutive days.The peripheral blood mononuclear cells isolated from 21 remission patients and normal controls were stimulated by rhGM-CSF and rhIL-4.The surface antigen of the dendritic cells was analyzed by flow cytometry.The level of IL-12p70 in the supernatant was detected by enzyme linked immuneosorbent assay.Result:In ITP patients,the expressions of both CD80 and CD86 in dendritic cell were significantly higher compared with those in normal controls [(51.60±13.47)% vs(36.03±15.43)%,(61.50±15.93)% vs(40.28±11.49)%,respectively,P0.05].After HD-DXM treatment,both CD80 and CD86 were decreased significantly without difference between HD-DXM treatment patients and normal controls(P0.05).The level of IL-12p70 in ITP patients was significantly higher [(67.52±14.43) pg/ml] than in controls [(39.78±10.03) pg/ml,P0.05].After the treatment,IL-12p70 decreased significantly to(43.90±8.49) pg/ml with no significant difference between ITP group and control group(P0.05).Conclusion:Dendritic cells and their cytokine secretion play an important role in ITP,and DXM may achieve its therapeutic effect on ITP by inhibiting the immunoreactivity of dendritic cells.
