育龄妇女转铁蛋白受体的比较研究

摘要 目的 比较育龄期妇女各项铁营养状况评价指标,研究血浆转铁蛋白受体(TfR)与其它铁营养状况评价指标的相关性.方法 以500名20~45岁身体健康的育龄期妇女为研究对象,比较各组血浆TfR、铁蛋白(SF)浓度,研究TfR与SF、血红蛋白(Hb)、总铁结合力(TIBC)及年龄相关性.结果 各年龄组间血浆Hb浓度、TIBC浓度没有差异,血浆SF浓度随年龄的增长逐渐降低,最低浓度为(42.7±27.5) μg/L,血浆TfR浓度随年龄增长逐渐升高,最高浓度为(29.1±7.1) nmol/L.多元线性回归分析表明,SF、Hb对TfR有影响.结论 血浆SF、Hb(即使SF、Hb在正常值范围内)均对TfR有影响,储备铁含量越低SF对TfR的影响越强. Objective To compare assessing indexes of iron status in child - bearing period women and look for correlation between TfR concentration and other measures of iron status. Methods Our study groups were healthy 20 - 45 years old child - bearing period women ( n = 500). The correlation of TfR and other indexes were studied. Results There were no difference of Hb and TIBC concentrations among diffenrent groups. With age increasing, SF concentration was decreasing and lowest eoncentration was (42.7 ±27.5 ) μg/L, and TfR concentrations were increasing and highest concentration was (29. 1 ±7.1 ) nmol/L in women aged 40 - 45 years old. By multiple regression analysis,the predictors of TfR concentration were plasma ferritin , hemaglobin. Conclusion Plasma ferritin, Hb, even within the normal physiologic SF, Hb, influence TfR concentrations. The lower the iron stores, the stronger the influence of ferritin on TfR.
