摘要
目的探讨医警联动机制的绿色通道在交通伤急救中的效果及护理配合。方法选择2001-2008年在沪-杭、杭-甬、杭-金-衢高速公路车祸致伤送入医院的交通伤患者。2001年1月至2004年12月经普通绿色通道由家属、目击者或120急救车送入医院者1807例作为对照组,患者送入医院后,由急诊室医护人员进行常规急救处理;2005年1月至2008年12月直接通过医警联动机制急救绿色通道送入医院者2590作为观察组。结果观察组患者从事故现场至医院和院内至手术室的时间短于均对照组,各项急救措施的到位时间早于对照组,预后及救治成功率和满意率均优于对照组,差异均有统计学意义(P<0.01)。结论医警联动机制的绿色通道为交通伤患者的抢救争取了时间,降低了伤亡率和伤残率,提高了患者的治愈率和满意度;良好的组织管理与护理配合保证了绿色通道的有序运行。
Objective To explore the effects and nursing cooperation of green channel of doctor-police linkage mechanism in emergency care of traffic injuries.Methods Patients with traffic accident injured traumas in Shanghai-Hangzhou,Hangzhou-Ningbo and Hangzhou-Jinhua-Quzhou expressways from 2001 to 2008 were admitted to the hospital and selected in the research.A total of 1807 patients who were admitted to hospitals through green channel by the family members,witnesses or 120 emergency vehicles from January 2001 to December 2004 were selected as the control group to receive routine emergency treatment.Totally 2590 patients who were admitted to hospitals through green channel of doctor-police linkage mechanism from January 2005 to December 2008 were selected as the observational group to receive doctor-police linkage mechanism and management.Results The time intervals in sending the patients from the accident scene to the hospital and from the hospital to the operation room were significantly shorter in the observational group than those in the control group（P0.01）;The present time of emergency measures was significantly shorter in the observational group than that in the control group（P0.01）.The prognosis and treatment success rates and satisfaction rate were significantly higher in the observational group than those in the control group（P0.01）.Conclusion The green channel with doctor-police linkage mechanism can gain the transport time for trauma patients in the rescue,reduce casualty rate and disability rate and increase the cure rate and patients＇ satisfaction.Good organization,management and nursing cooperation ensures the orderly operation of the green channel.
