摘要
目的探讨母亲妊娠期高血压疾病(HDP)对新生儿脑结构及电生理学的影响。方法对2006年6月-2009年6月在本院住院治疗的23例健康母亲分娩的新生儿(对照组)及33例HDP母亲分娩的新生儿(观察组)在出生1周内进行EEG、脑电地形图(BEAM)、颅脑超声或磁共振检查。2组在性别、胎龄、出生体质量及Apgar评分等方面均无统计学差异。EEG和BEAM检查使用8导记录电极,包括Fp1、Fp2、C3、C4、T3、T4、O1和O2。将EEG/BEAM检查结果均分为正常范围、轻度异常、中度异常、重度异常。应用SAS6.12软件分析HDP对新生儿脑结构及电生理学的影响。结果观察组19例有脑损伤,对照组6例有脑损伤,2组比较差异有统计学意义(P=0.0197)。观察组EEG异常12例,对照组EEG异常1例,2组比较差异有统计学意义(P=0.0052)。观察组BEAM异常12例,对照组BEAM异常1例,2组比较差异有统计学意义(P=0.0052)。结论HDP对新生儿脑结构及电生理学有显著影响。
Objective To explore the effect of hypertensive disorders of pregnancy(HDP)on the brain structure and electric physiology of newborn infants.Methods This study included 23 newborns whose mothers with normal pregnancy(control group)and 33 newborns whose mothers with HDP(observe group)in Nanjing Children's Hospital Affiliated to Nanjing Medical University Neonatal Ward from Jun.2006 to Jun.2009.The newborns in both groups were examined with electroencephalogram(EEG),brain electric activity mapping(BEAM)and echo or magnetic resonance imaging of brain within 1 week after birth.There were no differences in sexes,gestational ages,birth weights and Apgar scores between the 2 groups.The EEG/BEAM were recorded with 8 electrodes,including Fp1,Fp2,C3,C4,T3,T4,O1 and O2.The results were classificated into normal scale EEG/BEAM,minor,moderate or severe abnormal EEG/BEAM,the normal scale EEG/BEAM was categorized into normal EEG/BEAM,the others were categorized into abnormal EEG/BEAM.SAS 6.12 software was used to analyze the effect of HDP on the brain structure and electric physiology of the newborns.Results There were 19 infants with brain injury in observe group,and 6 infants with brain injury in control group.There were 12 infants with abnormal EEG in observe group,and 1 infant with abnormal EEG in control group.There were 12 infants with abnormal BEAM in observe group,and 1 infant with abnormal BEAM in control group.There were significant differences in brain injury,the abnormal EEG and abnormal BEAMs between the 2 groups(P=0.019 7,0.005 2,0.005 2).Conclusion HDP has significant effects on the brain structure and electric physiology of the newborns.
