摘要
目的探讨动力髋螺钉(dynamic hipscrew,DHS)治疗股骨粗隆间骨折的方法及效果。方法采用动力髋螺钉内固定治疗股骨粗隆间骨折51例,术后4～6周部分负重,6～8周依骨折类型及X线片所见情况,决定下地扶拐负重行走。结果随访时间6～18个月,骨折全部愈合,共发现髋内翻2例,患肢短缩外旋畸形4例。按照疗效判定标准,本组优34例,良12例,可5例,优良率90.2%。结论DHS治疗股骨粗隆间骨折,只要术中复位满意,操作时注意定位及方向感的把握,固定可靠,术后合理康复锻炼,临床效果满意。
Objective Exploring the the methods and efficacy of curing intertrochanteric fracture using DHS. Methods We adopted HDS to cure 51 cases with intertrochanteric fracture. Partial could be weight-bearing after 4 - 6 weeks. After 6 - 8 weeks, some could walk with walking stick depending on fracture type and X-ray film. Results After 6 - 18 months, all people＇ s fractures were healed. Only 2 cases were found coxa vara and 4 cases limb shortening external rotation deformity. According to the efficacy criteria, 34 cases of this group were excellent, 12 cases good, 5 cases passed. The rate of fine is up to 90.2 %. Conclusion We should pay attention to reset operation, location operation, direction grasping, fixation in surgery and the resumption exercise after. As long as we meet all the above notes in the surgery, the clinical results, using DHS to cure intertrochanteric fracture, will be satisfactory.
