摘要
目的运用电子束CT扫描进行正常组和缺血组的心肌灌注分析,建立心肌灌注检查常规,探讨临床诊断心肌缺血病变的新技术。材料与方法电子束CT检测心肌灌注28例,其中14例为对照组,另14例为病变组。所有心肌缺血病例皆经单光子发射体层扫描证实。取受检者心短轴,行心电触发,静注造影剂后6秒钟开始动态扫描(flowmode)。图像分析注意有无灌注缺损,并根据计算的组织灌注量或血流平均通过时间判断心肌缺血状况。结果每100g心肌平均血流量在对照组为132.6ml/min,缺血组为106.3ml/min。电子束CT检测心肌缺血病变的敏感性、特异性分别为96.4%和92.8%。缺血区灌注曲线的形态与病变的程度、性质密切相关。结论电子束CT动态扫描是临床进行心肌灌注分析的一项新技术,不但可以诊断心肌缺血病变,还可进一步作出定量、定性方面的评价。
Purpose To provide clinically a new method of myocardial perfusion (MP) study using electron beam computed tomography (EBCT) and to validate the criteria of normal MP in evaluation of ischemic heart disease(IHD). Materials and methods Twenty eight persons were examined with EBCT (Imatron C 150 ). Fourteen of them were controls and the others were patients with ischemic heart disease proved by SPECT. After administration of intravenous injection of contrast medium, ECG triggered scans were performed with flow mode at left ventricular short axis. All parameters of the curves were calculated and statistically processed. The authors put stress on the parameters of local perfusion defect and MBF (myocardial blood flow) or MTT (mean transit time) in dignosis of ischemic heart disease. Results The average myocardial blood flow(ml·min -1 ·100g -1 ) were 132.6 in controls and 106.3 in IHD. The sensitivity and specificity of dynamic EBCT in detecting the disease were 96. 4% and 92. 8% respectively. The shape of perfusion curve was related to pathophysiological changes.Conclusion Myocardial perfusion study by dynamic EBCT is clinically a new application which will be useful both for detection and for assessment of ischemic heart disease.
