摘要
目的探讨手脱套伤后急诊显微外科修复方法选择。方法采用甲瓣、腹部带蒂皮瓣、游离皮瓣、足趾移植等2～3种方法组合修复手脱套伤97例。结果移植甲瓣30例和足趾55例,82例成活,3例坏死;移植皮瓣共120例,113例成活,7例部分或全部坏死;二期通过换药和重新移植组织瓣创面得到修复。随访2月～5年,手功能满意。结论根据局部伤情和供区情况,审慎选择组合组织瓣移植急诊修复手脱套伤,可获得良好的临床效果。
Objective In order to explore the methods in repair of the skin degloving injury of the hand in emergency.Methods Combinations of 2-3 methods of wrap-around flaps, abdominal flaps, free skin flaps and toe transplantation were used in 97 cases in this paper.Results In the examination, 82 cases survived in 30 cases of wrap-around flaps and 55 cases of toe transplantation, and 3 cases putrescence. 113 cases survived in 120 cases of free skin flaps, and 7 cases putrescence. The wound were treated by dressing changes and retransfer the tissue flaps in secondary stage. Follow-up examination ranged from 2 months to 5 years, and the results of hand function were satisfactory.Conclusion According to the condition of local trauma and donor site , the satisfactory clinical results can be achieved by use of complexed tissue flap.
