摘要
目的：观察三维适形放疗同步化疗治疗中晚期食管癌的疗效和毒副反应。方法：69例不能手术或不同意手术中晚期食管癌患者随机分为治疗组（35例）和对照组（34例）。治疗组三维适形放疗同步DF方案化疗;对照组单纯三维适形放疗。两组放疗均采用常规分割,总DT66-70Gy。结果：治疗组与对照组总有效率（CR＋PR）分别为85.7%和64.7%（χ^2=4.99,P〈0.5）,一年生存率分别为80%和55.9%（χ^2=4.19,P〈0.5）,3年生存率分别为31.4%和23.5%（χ^2=0.39,P〉0.5）,不良反应主要表现为放射性食管炎和白细胞下降,多为Ⅰ、Ⅱ级。结论：三维适形放疗同步DF方案化疗治疗中晚期食管癌可显著提高患者的局部控制率和近期疗效,副反应轻。
Objective：To evaluate the therapeutic effetiveness and toxicity of three dimensional-conformal radiotherapy（3DCRT） combimed with chemotherapy for advanced esophageal cancer.Methods：Sixty-nine patients with advanced esophageal cancer either inoperable or who refused operation were randomly divided into two groups：35 cases in the treatment group received 3DCRT combined with chemotherapy DF（DDP and 5-FU）,34 cases in the control group received 3DCRT alone.Radiotherapy was given with conventional fraction,The total tumor does were 66-70Gy.Results：The overall response（CR＋PR）rates for the treatment group and the control group were 85.7%,64.7%（χ^2=4.099,P〈0.05）,respectively,The one-year survival rates for the two groups were 80%,55.9%（χ^2=4.619,P〈0.05）,respectively,The three-year survival rates for those two groups were 31.4%,23.5%（χ^2=0.539,P〉0.05）,respectively.The adverse reaction manifested mainly the radiation esophagitis and the leukopenia,but mainly were Grade 1 and Grade2.Conclusion：Three dimensional-conformal radiotherapy combined with chemotherapy DF （DDP＋5-Fu） for advanced esophageal cancer could significantly improve the local control rate and recent effective near-term rate and had slight side effect.
